Position Summary
The Behavior Health Care Navigator (BH CN) provides outreach to members to assist in identifying social determinants of health (SDoH) and barriers to care. The BH CN provides linkage to appropriate community resources and facilitates referral to other services through collaboration with Care Navigator – RN to improve member health care outcomes. This position reports directly to the Specialty Team Supervisor.
Responsibilities

• Collect information to identify psychosocial needs, (SDoH), and/ or barriers to care using established assessments and relays this information to the Care Navigator for care planning
• Participate in team meetings, multidisciplinary team meetings, in service trainings, and 1:1 meeting with Supervisor
• Facilitate linkage to appropriate community health/human services and assists with locating wrap around services available within the member’s community
• Provide Specialty Team updates at regular staff meetings
• Provide education and guidance on psychosocial aspects of kidney disease and the adult population and the effects of CKD progression on mental health (grief, depression, anxiety etc.)
• Provide intervention that may include education, guidance, and support in identifying and addressing barriers of SDoH (housing, finances, transportation and MH/BH needs)
• Collaborate with outside agencies to ensure continuity of care
• Perform other related duties as requested
• Document in HealthMap’s care management systems as appropriate
• Encourage adherence to provider treatment plan and Care Navigator plan of care
• Assist in telephonic outreach for purposes of engagement of members by client per referral from Care Navigation team
• Complete appropriate behavioral health assessments
• Accountable for individual metrics related to BH CN role as defined by the Care Services Director
• Work collaboratively with the Care Navigator or provider when barriers are identified

Requirements

• Masters in related Human Services/ MH, SW required; Active current License as LCSW, LMHC preferred
• Experience working in the community with knowledge of renal health, dialysis and/or community resources preferred
• 3 years’ experience in healthcare or human services agency setting
• Experience in managed care setting

• Proficiency with MS Office and automated database management systems (Excel, Access, PowerPoint)
• Excellent written and verbal communication
• Strong interpersonal skills required to motivate patients and others
• Ability to independently identify problems, clarify facts and organize information to identify trends, information gaps or need for additional information
• Strong organizational skills
