Hennepin Healthcare is an integrated system of care that includes HCMC, a nationally recognized Level I Adult and Pediatric Trauma Center and acute care hospital. The comprehensive healthcare system includes a 484 bed academic medical center, a large outpatient Clinic & Specialty Center, and a network of primary and specialty care clinics in Minneapolis and in suburban communities. Hennepin Healthcare has a large psychiatric program, home care and hospice, and operates a research institute, innovation center and philanthropic foundation.
SUMMARY
We are seeking a Social Worker I or II to join our MICU/SICU teams. This full time role will work days (8:00 – 4:30 PM), including a few weekends per year as well as one holiday per year.
Provides direct service to clients and their families. This includes counseling and seeking resources to answer unmet needs. The social worker also provides consultation services and in-service education programs related to the psychosocial aspects of health care to other professional staff members on the healthcare team.
Required COVID-19 Vaccinations for Employees

RESPONSIBILITIES
- Other duties as assigned
- Design and follow through on intensive long or short-term service/treatment plans
- Provide preventive and direct services to clients who have mental, emotional, or physical disabilities and other cases involving complex social and financial problems
- Interviews clients and performs psychosocial assessment to determine problems and appropriate types and methods of service/treatment
- May be involved in integrated, multidisciplinary or community initiatives and may be housed at community based sites including serving as a liaison between Hennepin County and communities, community organizations, schools, health professionals and service providers and identifying the needs and assist in resolving problems facing individuals and families regarding health and human service issues (including but not limited to housing, employment, financial assistance, and behavioral health).
- Prepare and maintain case records, case findings, correspondence, and reports
- Interpret and explain policies, regulations, and programs to clients and the public
- May provide work direction to social work paraprofessionals
- Make field visits to clients' homes
- May run/facilitate support/psychoeducational groups
- Prepare social histories with emphasis on psychosocial factors, adoption, or foster home placements
- In limited settings, may carry a selected caseload where there is a need for specialized continuing service/treatment
- May provide supportive counseling and crisis intervention (not psychotherapy) using social work methods under the supervision of LICSW
- Refer clients to appropriate community services and interpret social and emotional factors to others involved in treatment of clients
QUALIFICATIONS
Level I:
Minimum Qualifications:
- Bachelor's degree in Social Work
Knowledge / Skills / Abilities:
- Effective use of all Windows-based applications such as Outlook and Microsoft Office Suite products
- Ability to communicate with patients and families under sometimes stressful circumstances
- Experience with electronic health record or similar software program
- Demonstratable cultural competence with a variety of socio-economic, racial and other elements of diversity
License / Certifications:
- Unrestricted license by the Minnesota Board of Social Work as a Licensed Social Worker (LSW)
Level II:
Minimum Qualifications:
- Master’s degree in social work or Master’s degree in related discipline
Knowledge / Skills / Abilities:
- Experience with electronic health record or similar software program
- Demonstrable cultural competence with a variety of socio-economic, racial and other elements of diversity
- Ability to communicate with patients and families under sometimes stressful circumstances
- Effective use of all Windows-based applications such as Outlook and Microsoft Office Suite products
License / Certifications:
- Unrestricted license by the Minnesota Board of Social Work at one of the following levels: LGSW, LISW or LICSW
