Social Worker / Discharge Planner

A full-time Social Worker / Discharge Planner position is now available with Liberty Healthcare’s mental health program at the West Valley Detention Facility – a San Bernardino County jail located in Rancho Cucamonga, CA.

In this role, you will work onsite within the jail and provide basic behavioral health services to adult inmates who are experiencing with mental illness.

Your responsibilities will include:

• Delivering psychoeducational services to individuals and groups

• Facilitating skill building group sessions

• Facilitating contact with clients’ families and collateral contacts

• Overseeing clients’ transitions into the jail’s general population and transfers within the county jail system

• Discharge planning for inmate clients who are preparing for release from jail and reintegration into the community

• Coordinating community-based resources for inmate clients with parole officers, housing agents and outpatient therapists

• Collaborating with Psychiatrists, RN, LCSWs, LMFTs, LPCCs, Psych Techs and other ancillary clinicians and security staff

You will have some flexibility with your work schedule - though you will be required to work afternoons and evenings – within the parameters of 12:30 pm to 11:00 pm. You have the option to work a five day workweek with 8-hour workdays or a compressed four day workweek with 10-hour workdays.

As a full-time employee of Liberty Healthcare in this role, you can expect a starting in the salary range of $61.500 to $64,000. Liberty takes into consideration relevant work experience, degree, and other relevant factors when determining a new hire’s specific starting salary.

You can expect 30 days of cumulative paid time off each year plus benefits.

You are a good fit for this position if you possess the following:

• Bachelor’s degree in Social Work, Psychology or a related behavioral health or human services discipline

• At least one year of behavioral health case management experience – preferably within a jail, correctional facility, forensic hospital or comparable behavioral health setting

• Experience working with people who have serious and persistent mental illness

• Effective communication skills and the ability to write sound progress notes and reports
