YOUR LIFE AS A SOCIAL WORK CASE MANAGER

This full-time case management position in human services works during the day.

As a Social Work Case Manager with our nonprofit, you're responsible for providing substance abuse assessment and case management services to formerly incarcerated clients enrolled in our IMPACT program. You orient clients about program goals, objectives, rules, regulations, client obligations, rights, and program operations. Using good clinical judgment, you identify barriers preventing clients from obtaining employment as well as determine the appropriate treatment needs and level of care. As a member of our interdisciplinary team, you collaborate with other staff and function as a liaison to our client's families and other agencies.

Additionally, you conduct thorough assessments and use a motivational interviewing process to develop individualized person-centered transition plans. You determine the appropriate level of care for clients already in treatment, including relapse prevention, and review their treatment history using available resources. As required, you facilitate groups and seminars on substance use topics. You also identify available vacancies in appropriate treatment programs and make appointments for initial interviews while minimizing disruption to clients and their families.

Along with other community-based providers, you participate in case conferences regarding our clients. You ensure that client records are accurately maintained and that client information is reported in accordance with external regulatory standards as well as agency policies. When needed, you provide crisis intervention techniques and generate referrals to mental health services. Your dedication to your clients is apparent in all the hard work you do. What We Need From You

• Strong writing skills
• Competent in writing goal-based and person-centered treatment plans and progress notes as evidenced by the successful completion of a writing sample
• Bachelor's degree in social work or a relevant field such as psychology, vocational rehabilitation, or occupational rehabilitation
• Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
