We are a non-profit long term care facility that offers independent living, assisted living, and skilled nursing care. At Sharon Towers our team member's number one responsibility is to deliver compassionate resident care every time!


The Social Services coordinator assist the Social Services Manager in planning, organizing, and implementing the overall operations of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, established policies and procedures of Sharon Towers, and as directed by the Social Services Manager, and/or the Healthcare Administrator, to assure that the medically related physical, mental, emotional and social well-being of the resident is met/maintained on an individual basis.


In compliance with new federal requirements for Long Term Care, Proof of COVID-19 vaccination is required for all employees.

Qualifications


  • A bachelor's degree in social work in a related human services field is strongly preferred.
  • A minimum of six months experience in a human services setting working directly with patients or clients is required.
  • Must have excellent communication skills.
  • High school diploma is required.


ESSENTIAL DUTIES AND RESPONSIBILITIES Include, But Not Limited To


  • Assists with the admission process, as directed, to facilitate a positive move in experience for residents to healthcare or assisted living.
  • Plays a key role in the Interdisciplinary Care Plan Team, and, as the care plan coordinator, is responsible for scheduling and coordinating quarterly care plan meeting dates and times with the families, residents and the Interdisciplinary Team.
  • Interviews residents/families to obtain social histories for the completion of initial/annual assessments and care plans.
  • Provides a copy and explains Resident Rights to residents and/or P.O.A. upon admission.
  • Other duties as assigned.
  • Participates in the planning, development and implementation of social service care plans and resident assessments upon admission, quarterly, and as needed.
  • Participates and assists with coordination of transfer between levels of care.
  • Coordinates with resident and family in planning of social care plan objectives and goals.
  • Responsible for addressing any resident changes in status related to social issues and communicate effectively with the Interdisciplinary Team to implement effective interventions.
