The Scheduling Coordinator is responsible for assisting in the coordination of all clinical services provided. The Scheduling Coordinator coordinates the scheduling of patient care visits for both new patients and existing patients with the appropriate clinical staff. The Scheduling Coordinator is responsible for arranging coverage of patient care visits for sick calls, vacations, etc. The Scheduling Coordinator facilitates the weekend and holiday patient visit schedules. The Scheduling Coordinator is responsible for data entry of new referral demographic information, assignments, and coordination notes, retrieving information from hospital and physician electronic records. The Scheduling Coordinator is responsible for ensuring that patients are assigned in a timely manner. The Scheduling Coordinator retrieves all telephone messages related to scheduling of staff and patients. The Scheduling Coordinator is responsible for communicating with the Patient Care Coordinators on staffing and coverage issues, reports trends and/or patterns. The Scheduling Coordinator works with staff coordinating referrals and initial visit assignments.
Minimum Qualifications:
Education: High school graduate.
Experience: Minimum of three (3) years experience in a home health office setting or as a unit clerk, a scheduler, or a referral coordinator.

Demonstrated knowledge of medical terminology. Demonstrates appropriate verbal and written communication skills. Knowledge of computers, scheduling programs, and equipment such as telephones, fax, and copiers is required. Working knowledge of home health or hospice.
