Job Description
As leaders in suicide prevention and mental health care, Vita Health is on a mission to improve access to high quality mental health care for youth ages 12 - 24, families, and other adults. Our suicide prevention programs, general therapy, and psychiatry services are offered through our telehealth platform. We are a group of compassionate and dedicated clinicians focused on meeting our patients where they are and helping improve patient's quality of life through the use of evidenced based mental health care, including proprietary suicide prevention interventions and an app that offers support in between sessions as well as outcome monitoring. We partner with primary care providers, colleges, and major health systems and insurance plans to ensure our services are accessible, affordable, and timely. You love to help people, and so do we! We are seeking a Care Coordinator in Central or Eastern time zone to join our team! This role is fully remote.


As a Care Coordinator, you are an intricate part of a dynamic team of like-minded individuals who work together to provide quality care to those needing Behavioral Health services. Our Care Coordinators work together to perform daily duties such as data entry, scheduling, and patient information management. They communicate efficiently and effectively both verbally and electronically through several programs such as Athena Health (EMR), Dialpad, and Google Suite. You will connect with your team members, Vita employees, and leads/managers through Google Meet. Care Coordinators must be comfortable working individually and with their team to accomplish daily duties and tasks in a remote environment. Your "office" or workspace must be quiet, free of distractions, and meet HIPAA compliance standards. We are excited you are considering a position with Vita Health, and thank you for applying!


Responsibilities


  • Able to maintain a HIPPA Compliant, professional, quiet/non-disruptive work environment
  • Documents interactions with patients across various mediums.
  • Accurately enters data regarding patient notes, referrals, and demographics.
  • Effectively communicates Vita Health services and offerings to patients, parents, and referrers by answering inbound calls and returning outbound messages.
  • Other duties as assigned.
  • Acts as a liaison for patients with other organizational verticals (i.e. Clinical, business development, etc.)
  • Schedules, reschedules, and cancels patient appointments, as needed.
  • Responds to patient and Clinician inquiries via phone, text, email, and Athena requests.


Compensation rate: $18 per hour


Minimum Qualifications


  • Relevant certificates and or degrees
  • Pertinent work experience
  • High School Diploma

Background / Experience


  • Medical Records: 1 year (preferred)
  • Excel: 1 year (preferred)
  • Microsoft Office: 2 years (Preferred)
  • Behavioral Health or Medical Office setting (Remote experience a plus)
  • Google Suites: 1 year (preferred)
  • Experience with Athena and or additional EMR/Customer Management Programs: 2 years
  • Billing and Insurance Verifications: 1 year (preferred)
  • Customer Service: 2 years (Preferred)

  • Prevent entities not employed by Vita Health/Vytyl Health Management, Inc. from having access to verbal, written, and or electronic communication and or correspondence before, during, and or after work hours.
  • Ability to secure and or lock any work area/office space, devices containing PHI, and the ability to secure, lock up, and or shred any physical, paper, and or handwritten documentation that may contain PHI
