The Transgender Clinic for Affirmation and Reproductive Equity (CARE) Nurse Navigator is the cornerstone of clinical adhesiveness and flow between disciplines. They play a primary role in patient satisfaction by providing continuity of care that is timely and accessible. The TransCare Nurse Navigator will have a critical role in the safe management of these complex patients by shepherding them through hormone surveillance, pre- an post-operative follow-up, and monitoring of routine health screening.
Essential Job Functions:
- Assure that clinical plan from office visit is: scheduled, completed, interpreted by MD, disclosed to the patient and documented in EMR
- In clinic bedside assistance for visits and procedures (e.g. pap tests, injection teaching, wound examination/drainage, etc.)
- First contact patients to answer clinical and logistic questions. This includes active management of routine medication refills, response to normal laboratory results, and collection of pre-surgical letters of support, and response to EPIC in-box and clinical phone questions/concerns. Escalate to provider when necessary or coordinate a clinic visit when needed
- Clinic templates pre-screen for correct provider, correct timing/appropriateness for appointment, items needed for appointment
- May be responsible for direct patient care
- Consult with clinic MAs to optimize EPIC documentation and clinic workflow
- Develop a personal relationship with patients in long-term follow-up in Trans Care, particularly those on gender-affirming hormone therapy and/or receiving gender-affirming surgery
- Develop (with clinic admin) and implement a clinical tracking system to assure that patients followed in Trans Care remain on schedule for screening tests, surveillance, and office visit
- Answer clinical questions escalated to RN by other clinical support staff (MAs, RNs)
Required Qualifications:
- Minimum of two years clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients.
- Experience or willingness to learn Core Concepts of sensitivity toward Gender Diverse (transgender, non-binary, gender non-confiming) patients
- Familiarity with screening and survellance guidelines for gender-affirming care
- General understanding of the spectrum of gender-affirming care including primary care, hormone, and surgical options
- Registered Nurse with a current Illinois license. Bachelor's of Science in Nursing referred.
- Must have general computer skills and be adaptable to changing working situations and work assignments.
- Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians.
- Certified Nurse in Specialty area preferred or in process.
