Job Description
The RN provides, coordinates and evaluates effectiveness and outcomes of care to clients presenting for medical/ nursing based services at the VNA Health Center clinic sites (on and off clinic sites). These clinics and services may include:
  • Immunization Clinic – assessment, administration and counseling for the ACIP recommended vaccines for clients from infancy through adulthood.
  • Child Health Clinic – care to children in need of routine preventive, and for acute and chronic illnesses. This may include: assessment and counseling in all aspects of child health (physical, developmental, emotional, social, dental and nutritional); immunization assessment, administration and counseling; and performing procedures related to care (e.g. anthropometric measurements, vital signs, and Waived Testing.
  • Women’s Wellness Clinic – care to women in need of health maintenance and promotion services, family planning, STD counseling, as well as care for acute and chronic illnesses. This may include: assessment and counseling in all aspects of women’s health; immunization assessment, administration and counseling; and performing procedures related to care (e.g. anthropometric measurements, vital signs, and Waived Testing
  • Primary Health Care – comprehensive primary medical care for clients of all ages across the health care continuum. Includes performing procedures related to care (e.g. vitals, treatments as ordered by the MD/NP…), assessment of patients’ needs for counseling, education and referrals, pregnancy options counseling, and family planning care and education.
  • Prepares nursing, medical and lab areas with medical supplies; emergency kits; vaccine; and educational and referral materials as appropriate for specific clinic services.
  • Reviews thoroughness of Client Registration/ Consent and other consents specific to services to be provided.
  • Obtains complete client and family health histories if appropriate to specific clinic services.
  • Assesses client needs, develops and coordinates an individualized plan of care, including involving the client/family in the development of the client’s goals and collaboration with other health care professionals as necessary.
  • Initiates nursing interventions identified in the plan of care, per applicable Standing Orders and Protocols, evaluating effectiveness and outcomes of care and revising as needed.
  • Administers medications as ordered by the clinic physician or nurse practitioner according to applicable protocols, policies and procedures. This includes medications related to family planning and emergency contraception.
  • Educates/counsels client (or parent/guardian, if client is a child) specific to identified needs, plan of care and clinic requirements as described in the appropriate clinic policies and procedures. (e.g. Family Planning Program, IBCCP Program, Immunization Program)
  • Initiates referrals to other health care services as needed and completes follow up on referrals per applicable protocols, policies and procedures.
  • Completes client clinical record documentation specific to clinic services provided, according to applicable policies and procedures, and in a timely manner.
  • Maintains safe handling, administration and storage of vaccines and medications.
  • Documents administration of medications and vaccines on the appropriate inventory log and EMR.
  • Understands and applies exposure control procedures as described in the VNA Infection Control Policy and Procedure Manual.
  • Understands medical emergency procedures and the location of the emergency kit(s)/supplies in all clinic settings. Performs routine emergency kit(s)/supplies checks prior to clinic operation.
  • Communicates with other health care providers regarding client continuity of care, after appropriate consent for release of information provided by the client; and assists in obtaining/completing activities related to requests for medical records.
  • Notifies client and initiates follow up on abnormal lab results per applicable protocols.
  • Maintains professionalism and adheres to client confidentiality in all VNA Health Center practices.
  • Develops and maintains a cooperative working relationship with the Clinical Services Management staff, other nurses, clinicians, nutritionists, Certified Nurse Assistants and support staff.
  • Identifies and participates in continuing education program/activities including completion of annual competencies to enhance professional growth and ensure quality of care.
  • Participates in ongoing Quality Improvement activities for the VNA Health Center Clinic programs.
  • Assist in the planning for and scheduling of clinics.
  • Monitor laboratory/mammography results and the initiation of treatment or referral for abnormals in a timely manner.
  • Assist in the supervision of the CNA staff and other nursing staff during clinic to ensure proper flow, efficient operation and clean up of the clinic.
  • Monitor inventory of medical/nursing supplies, pharmaceuticals and client educational materials and order as needed to maintain efficient/cost effective clinic operation.
  • Participate in VNA Health Center committees, as assigned (eg. Education, Infection Control, Forms, Quality Management…).
  • Assist in the planning and implementation of new programs or services.
  • Monitor the follow up of referrals.
  • Assist in the observation of CNA staff, PRN nurses and other staff nurses for the purpose of input for job performance evaluation.
  • Assist in the orientation and in-servicing of staff.
  • Assist in the development of forms, client educational materials and in the development or revision of policies, procedures, protocols and standing orders.
  • Assist in staff and community education; and outreach.
  • Nurses may be identified and given the additional responsibilities to:
    • Assist in the supervision of the CNA staff and other nursing staff during clinic to ensure proper flow, efficient operation and clean up of the clinic.
    • Assist in the orientation and in-servicing of staff.
    • Assist in the observation of CNA staff, PRN nurses and other staff nurses for the purpose of input for job performance evaluation.
    • Assist in activities needed to help ensure the safety and security of the VNA Health Center clinic clients, staff and facilities.
    • Monitor laboratory/mammography results and the initiation of treatment or referral for abnormals in a timely manner.
    • Monitor the follow up of referrals.
    • Maintain pharmaceutical/vaccine inventory and the supplies needed to dispense; and maintain the pharmaceutical dispensing logs and vaccine accountability.
    • Assist in the planning for and scheduling of clinics.
    • Assist in the development of forms, client educational materials and in the development or revision of policies, procedures, protocols and standing orders.
    • Assist in the completion of clinic reports.
    • Assist in staff and community education; and outreach.

    • Assist in the planning and implementation of new programs or services.

  • Assist in activities needed to help ensure the safety and security of the VNA Health Center clinic clients, staff and facilities.
  • Maintain pharmaceutical/vaccine inventory and the supplies needed to dispense; and maintain the pharmaceutical dispensing logs and vaccine accountability.
  • Assist in the completion of clinic reports.
  • Ability to work all clinic hours as assigned.
  • Able to work effectively and efficiently in the EHR.
  • Ensures the patient education model of care is fully implemented in their respective areas of practice.
  • Takes responsibility for the implementation of policies and procedures for Joint Commission Accreditation.
  • Familiarizes and complies with all VNA policies and procedures.
  • Meets standards of behavior expectations.
  • Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment.
  • Other duties as assigned.

  • Nurse registered to practice in the State of Illinois, BSN preferred.
  • Maintains current CPR certification.
  • Previous experience in community health is preferred.
  • Education, training and/or experience in child health, immunizations and/or women’s health (including family planning and STDs) specific to clinic assignment (Child Health, Adult Health, Family Planning, Immunizations …).
  • VNA Health Center health policies and requirements.
