As a Home Health Care RN with us, you'll play a pivotal role in ensuring the well-being of our patients through in-office work, while adhering to policy and procedures, Title 22, and CMS regulations. Your role will involve conducting initial assessments, providing comprehensive care plans, and fostering continuous communication with patients, their families, and healthcare professionals.
Here's What You'll Be Doing
Conducting in-depth assessments of patients and their families to determine their specific home care needs, considering their medical history and current conditions.
Creating personalized care plans based on nursing diagnoses, incorporating therapeutic, preventive, and rehabilitative actions.
Collaborating closely with the patient and their family in the planning process, ensuring their active involvement and satisfaction.
Regularly re-evaluating patients' nursing needs and providing counseling and health instructions as required.
Overseeing LVN's and home health aides to ensure the highest quality of care delivery.
Serving as a proactive case manager, visiting patients post-hospitalization and updating care plans accordingly.
Identifying and implementing discharge planning needs to ensure a smooth transition for patients.

Qualifications We're Looking For
Valid RN License and a minimum of one year of Home Health experience.
Bilingual proficiency is a plus.
Comfortable working with State and Joint Commission standards.
Familiarity with Medicare and other private insurances.
