This position reports to the Director of Nursing and Primary Care Clinical Services and works as part of an interdisciplinary team providing administrative services as well as field-based patient care to Yolo County residents with unstable housing. The RN has a leadership role developing daily schedules, participating in strategic planning, and interfacing with community partners; the RN also provides direct patientcare and provides oversight for program support staff.
CommuniCare Health Centers is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, behavioral health, and outreach services to the low income, multi-ethnic, uninsured, and underinsured in the communities we serve.
HOURS/SITE: Full-time, 40 hours/week, Monday through Friday at clinical sites in Davis, West Sacramento, Woodland, and multiple other locations throughout Yolo County. Some evening hours required.
RESPONSIBILITIES:

Leadership
- Manages inventory of supplies in conjunction Mobile Medicine team.
- Maintains communication with incoming and outgoing referrals with internal and external partners.
- Manages communications channels to include, but not limited to, e-mails, voicemails, incoming/outgoing phone calls.
- Provides leadership to the Mobile Medicine team through clinic flow, roster management, and quality improvement initiatives.
- Assures efficient patient flow by ongoing assessment of team’s schedules (daily, weekly, and seasonal). Works with staff to optimize field services.
- Provides oversight of medical assistants and health coaches as they perform screenings for blood pressure, blood sugar and infectious disease.
Direct Clinical Care
- Performs diagnostic and laboratory tests including but not limited to iSTAT, A1c, phlebotomy, urine collection for pregnancy and culture etc. Supervises and oversees quality assurance of these testing services performed in the field.
- Utilizes the nursing process (assessment, planning, intervention, and evaluation) in patient care and daily activities. Should be fluent with interdisciplinary communication and SBAR reporting format.
- Transmits new, updated, and refilled prescription orders to pharmacies on behalf of prescribing providers.
- Furnishes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions, within scope of practice.
- Administers injections and diagnostic test/referrals as ordered, including immunizations, completing lab and x-ray requisitions and other services ordered by the providers.
- Performs patient care including but not limited to wound care, birth control visits, blood pressure checks, medication refills, etc. as needed.
- Provides direct one-on-one patient education and oversees provision of patient education by Medical Assistants. Participates in group education as needed.
- Provides and manages patient care under the direction of Mobile Medicine providers, including physical evaluations, assessments, and diagnoses as appropriate. Consults with team providers as needed.
Care Coordination
- Provides follow-up to patients recalled for abnormal labs/tests.
- Assists with coordination of specialty clinics, such as orthopedics, pediatrics, dermatology, telemedicine, etc.
- Processes clinical patient information for providers in patient electronic health records (EHR).
- Performs independent face-to-face or telephone triage using written triage protocols.
- Assist with coordination of care to other CommuniCare services, including dental care, behavioral health, diabetes education, etc.
- Assists with meeting the needs of patients with complex medical problems, including facilitating referrals and follow-up care as needed.
- Accepts and correctly transcribes verbal or telephone orders from clinicians.
- Attends multidisciplinary, clinic and general CommuniCare meetings as required.
Additional duties
- Participates routinely in quality improvement projects using PDSA model.
- Assists with driving the Mobile Medicine van to clinical sites.
- Other duties as assigned
MINIMUM POSITION REQUIREMENTS:
Education/Licenses: Completion of an accredited RN Program. Current California RN license and good standing with the Board of Registered Nurses required. Current CPR certification required.
Experience: Knowledge of nursing process, scope of practice and standards of care. Previous nursing experience in a medical office, outpatient clinic or hospital strongly preferred. Nursing experience working with unhoused populations also preferred. Demonstrated ability to assume responsibility, to work well on a team and problem solve. Previous experience with continuous quality improvement process and with safety, environmental and/or infection control standards strongly preferred. Previous leadership experience strongly preferred. Experience with or ability to learn utilization of electronic health records. Demonstrated commitment to high quality health care for low-income populations.
Special Skills/Equipment: Basic computer proficiency in Windows environment required. Effective oral and written communication skills essential. Bilingual Spanish/English preferred.

Requires a moderate to high level of mobility, dexterity, and flexibility to perform essential functions. The employee is occasionally required to walk, bend, reach with hands and arms, and to lift 25 lbs. Must be able to sit or stand for long periods of time. Close vision is required. Work is performed outdoors during inclement weather, temperatures range from low 30’s to over 100F. Anticipate uneven ground, hiking into camps, muddy conditions, exposure to unsanitary encampments. Travel is an essential part of the work. Anticipate providing services throughout Yolo County. Vehicle provided and mileage reimbursed when using personal automobile. Ability to bend and move arms and hands in sufficient range of motion to complete physical assessment and type chart notes. COVID VACCINATION REQUIRED.
PAY RATE:
- May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).
- The pay range for this position is $83,928/yr.-$125,944/yr. Starting pay within this range will vary depending on the job-related knowledge, skills, and experience.
