Registered Nurse Ii-Iv - Enhanced Care Management Team

The County of Riverside - Riverside University Health System- Behavioral Health is currently recruiting for Registered Nurse II/III/IV for our Enhanced Care Management (ECM) Team. Candidates with experience working with substance use, and/or mentally ill are encouraged to apply. Bilingual is preferred.
Incumbents in this position will be responsible for the following:

• The RN will attend Nurse Manager meetings, including at the Integrated Health Care sites, and coordinate the dissemination of information, both internally and externally to the staff regarding physical health care of clients.
• The RN can ensure the Nursing Department is prepared for internal/external audits by maintaining appropriate reports, licensure, logs and nursing policies and procedures.
• The RN will coordinate with supervisor and other ECM team members BHSII to ensure compliance with all contract obligations and to ensure that IEHP documentation is completed in a timely manner and to specifications.
• The RN will be responsible for the daily functions and oversight of the ECM Team, including but not limited to, providing nursing services to the clients, preparing and managing correspondence, reports, documents, logs (compliance logs) and orders pertaining to pharmacies, psychiatrists and medications and monitor and track nursing licenses.

Enhanced Care Management (ECM)
Enhanced Care Management (ECM) will address the clinical and non-clinical needs of high-need, high-cost Medi-Cal members through systematic coordination of services and comprehensive care management. Enhanced Care Management (ECM) is part of a broader population health system design within CalAIM, under which Medi-Cal providers will risk stratify their enrolled populations and offer a menu of care management interventions at different levels of intensity, with ECM at the highest intensity level.
