Quality Improvement Liaison, Lvpho

Job Summary:
The LVPHO Clinical Quality Improvement Liaison will coordinate clinical performance/value improvement activities for the LVPHO value-based contracts. This individual will work directly with practitioners, and their administrative, office and hospital staff colleagues to support them in clinical improvement activities that will allow them to reach clinical improvement goals for the beneficiaries/Members in those plans. This individual will also serve as the main clinical liaison for a group of practices and hospital departments in our participating hospitals
