- Ensures accurate patient demographic and billing information is correctly entered into the District’s software system(s).
- Collaborate with the Population Health & Innovation team to support value initiatives and patient coordination among the health care setting.
- Provides follow-up phones calls with patients in need of support.
- Assumes primary responsibility for the accurate and timely scheduling of all patients that are due for a wellness visit.
- Works closely with all patients and payers to ensure maximum benefit utilization for required care and services.
- Assist with data collection and documentation to support value-based needs.
- Support SPMC patients with coordination of primary care services.
- Performs other related duties as assigned.
- Essential Job Function
- Support with report filtering, managing, and disseminating as needed.
- Researches and analyzes alternatives for improving workflows. Develops “best practice” recommendations by partnering with other team members in examining data and identifying trends.
- Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Consistently demonstrates SPMC values and provides all patients/customers with an excellent service experience.

- *Supports SP mission, vision, and values: Develops awareness of departmental contribution to mission, vision and strategy. Participates in department strategies and tactics.
- 1 year of experience in referrals, registration, or patient scheduling
- Knowledge of medical terminology, preferred
- 1 year of experience in a clinical setting handling patient information
- High school diploma or equivalent required, Associate Degree preferred.

- Proficient in the use of current technology and software, including intermediate to advanced Microsoft Office skills (Word, Excel, Outlook).
- Proven ability to handle confidential information with discretion. Ability to develop and maintain accurate documentation and records.
- Demonstrated teamwork and collaboration skills. Strong customer service and interpersonal skills; ability to maintain composure and interact calmly and professionally with co-workers, providers, patients and visitors in a fast-paced, high-stress environment.
- Professional and effective written and verbal communication skills. Ability to identify and employ communication strategies appropriate to the audience. Grammar and punctuation skills as needed to compose professional documents.
