Job Summary


The Patient Coordinator is responsible for engaging and reaching out to patients, scheduling visits at Summit as appropriate, and providing needed information to ensure that they are meeting their health goals while coordinating with primary care, and documenting outreach outcomes. This position accurately and safely works with patients and the Population Health & Innovation team to coordinate and schedule care. They will ensure patients are provided with quality and compassionate customer service from the time of referral through follow-up care. Specifically, the patient coordinator will support new population health initiatives to improve health outcomes.

Job Duties and Responsibilities


  • Ensures accurate patient demographic and billing information is correctly entered into the District’s software system(s).
  • Collaborate with the Population Health & Innovation team to support value initiatives and patient coordination among the health care setting.
  • Provides follow-up phones calls with patients in need of support.
  • Assumes primary responsibility for the accurate and timely scheduling of all patients that are due for a wellness visit.
  • Works closely with all patients and payers to ensure maximum benefit utilization for required care and services.
  • Assist with data collection and documentation to support value-based needs.
  • Support SPMC patients with coordination of primary care services.

  • Performs other related duties as assigned.

  • Support with report filtering, managing, and disseminating as needed.
  • Researches and analyzes alternatives for improving workflows. Develops “best practice” recommendations by partnering with other team members in examining data and identifying trends.
  • Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Consistently demonstrates SPMC values and provides all patients/customers with an excellent service experience.

Required Education And Experience


  • 1 year of experience in referrals, registration, or patient scheduling
  • Knowledge of medical terminology, preferred
  • 1 year of experience in a clinical setting handling patient information
  • High school diploma or equivalent required, Associate Degree preferred.

  • Proficient in the use of current technology and software, including intermediate to advanced Microsoft Office skills (Word, Excel, Outlook).
  • Proven ability to handle confidential information with discretion. Ability to develop and maintain accurate documentation and records.
  • Demonstrated teamwork and collaboration skills. Strong customer service and interpersonal skills; ability to maintain composure and interact calmly and professionally with co-workers, providers, patients and visitors in a fast-paced, high-stress environment.
  • Professional and effective written and verbal communication skills. Ability to identify and employ communication strategies appropriate to the audience. Grammar and punctuation skills as needed to compose professional documents.
  • Strong organizational skills; ability to prioritize, and reprioritize, work as needed to meet objectives.
  • Ability to work independently using good judgment, problem-solving, organization, and prioritization skills to complete assignments in a timely, accurate, and effective manner.


This is a union position with a pay range of $19.49 - $27.20 per hour, depending on experience.
