Palliative Social Worker Jobs

The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.

Scope
Provides social services to patients in the care site and/or assigned unit.

Job Essentials

• Provides community resources for psychosocial, financial, and educational needs ofpatient and family. Collaborates with outside agencies. Participates in communityactivities, community-wide inservices and training to promote psycho-social health.
• Participates in interdisciplinary rounds and multidisciplinary care planning. Communicates and collaborates with medical, interdisciplinary team staff, patient, family, and community agencies to provide continuity of care.
• Provides education on palliative and hospice care, and coordinates referrals when indicated.
• Assists in the development, implementation and annual review of policies, procedures and protocols related to Palliative Social Work.
• Provides education and information on advance care planning and advanced directives. Serves as a resource in the areas of financial support, disability eligibility, workplace issues, transportation barriers, mental health, substance abuse, domestic violence, child and elder abuse, and housing resources.
• Provides continuing care assessments and reassessments of patients and their families to assist in development of their treatment and survivorship needs. Identifies psychosocial needs and access to appropriate services and resources.

Minimum Qualifications

• Current State Licensure as LCSW
• Master's Degree in Social Work.

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).

May be expected to stand in a stationary position for an extended period of time.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
