Outreach Representative Jobs

SUMMARY OF POSITION:

The Community Health Worker will primarily be working out in the community with specific target populations. He/she will conduct public education activities to raise awareness about health issues. The position will also assess community needs and refer them to the appropriate resources. Facilitate linkages for community members to CPP programs for primary care. This postilion will help community members develop health management plans and goals and follow up to ensure that have completed required or recommended actions. In addition, motivate the community members to be active and engaged participants in their health.

EDUCATION AND REQUIREMENTS:

High School Diploma or GED, Bachelors preferred.

Two years of work experience.

Experience with youth enrichment services and youth development; experience in mentoring youth and curriculum development; and in clinical research.

Bilingual (English/Spanish) preferred.
