Nurse Practitioner (NP) Trauma Surgery Salem Hospital / MGH

Under direction of the supervising surgeon, Sr. Administrative Director and the Trauma Nurse Director Division of Trauma, Emergency Surgery and Surgical Critical Care (TESSCC) and in collaboration with the Practice Manager of the Danvers Surgery Practice the Nurse Practitioner (NP) provides clinical care with a primary focus on facilitating excellence in patient care delivery, providing operating efficiencies and insuring compliance with hospital and regulatory policies and practices. Functions under clinical direction of the supervising surgeon. The NP will assist the Sr. Administrative Director and the Danvers Practice Manager with daily operations, issues and decision making to support the highest quality of patient care and clinical operations with the Salem Hospital Acute Care Surgery (ACS) team. Collaborates with supervising surgeons to support clinical compliance and ensure operational needs are met.
The NP is responsible for participation in sign-out rounds at Salem Hospital with the General Surgery and Trauma/Acute Care Surgery teams and to assist MGH surgical resident staff with follow up care planning and coordination of timely discharges of surgical patients from Salem Hospital (approximately 50% of total effort). In addition, the NP is responsible for inpatient and clinic support for the Salem ACS team and triage/clinical coordination of the ACS patients.
Clinical Practice
- Documents plan of care in patient’s medical record and clinical notes.
- Collaborates with nurse managers, clinical nurses and medical staff to plan, implement and evaluation pertinent change in practice and health care delivery.
- Collaborates with unit staff and physicians to facilitate a comprehensive plan of care and coordinate resources.
- Performs (oversees/assures completion of) specialized procedures particular to the select population mutually agreed upon with collaborating physician.
- Identifies system-wide factors contributing to undesirable outcomes, such as complications, increased length of stay and overutilization of services.
- Maintains and practices in accord with practitioner guidelines per state and hospital regulations.
- Utilizes advanced nursing knowledge to identify, prevent or solve complex and/or recurring patient care problems.
- Has a sound knowledge of nursing, medical and scientific principles and appropriately applies those principles to practice.
- Monitors patient outcomes in accordance with defined standards of patient care.
- Collaborates with nurse managers, clinical nurses and medical staff to plan, implement and evaluation pertinent change in practice and health care delivery.
- Identifies system-wide factors contributing to undesirable outcomes, such as complications, increased
length of stay and overutilization of services. - Facilitates patient movement through the system and efficient use of resources through interdepartmental collaboration.
- Maintains patient's right to privacy, ensuring patient dignity in the provision of care.
- Orders, interprets and evaluates appropriate lab and diagnostic tests.
- Follows and manages patients (in collaboration with MD) enhancing continuity of care.
- Facilitates patient movement through the system and efficient use of resources through interdepartmental collaboration.
- Performs medically delegated aspects of patient care as agreed upon by supervising physician in according to practice guidelines.
- Performs and records the patient history and physical exam and discusses findings with the physician.
- Writes orders which are in accord with current guidelines.
Clinic Responsibilities
- Identifies personal learning needs and goals and develops a plan to meet them.
- Serves as a clinical resource for staff in the Salem Clinic for clinical questions, issues and emergencies.
- Interact in a courteous manner with physicians, nurses, and administrative staff within and outside of the MGH.
- Working in tandem with the surgeons, triages patient communication for the practice, evaluates patient outcomes and implements quality patient care delivery systems. Ensures closed-loop message handling.
- Possess a global awareness of the daily schedule of the North Shore surgeons, which includes strategizing and implementing changes to the clinical schedule, absorbing patient care as needed/appropriate.
- Maintains and develops professional practice through continuing educational programs and appropriate learning experiences.
- Serves as clinical point person for patient education and questions regarding procedures.
- Triage in-house, clinical care and patient calls and patient gateway messages as first line of contact for surgeons and ensures follow-up as necessary in collaboration with surgeon.
- Utilizes resources efficiently to facilitate optimal patient care.
- May receive and manage in-hospital pages for resident attending staff during OR hours.
- Adheres to requirements for mandatory continuing education and certification in the field of advanced practice nurse.
- Participates in sign-out rounds at Salem Hospital with the General Surgery and Trauma/Acute Care Surgery teams and to assist MGH surgical resident staff with follow up care planning and coordination of timely discharges of surgical patients from Salem Hospital each weekday.
- Communicates the plan of care and other pertinent information to/from other healthcare providers as requested.
- Assist with patient care, including supervision of pre-op/post-op needs (e. g. labs, EKGs, medication lists, consents, H+Ps) and coagulation management.
- Participates in the implementation of new clinical techniques/routines.
- Provides optimum quality care in all patient care provided. See patients both independently and with surgeons during office clinics, and provides the highest quality NP care through assessment, planning and evaluation for clinic patients.
- Reviews patient information in Epic pertinent to pre-op and post-op patient care and enters clinical documentation to Epic as appropriate.
Salem Hospital Outpatient Clinic
- Provides cross-coverage with Salem and/or Danvers Outpatient Surgery Suite APPs as necessary.
- In cooperation with the Practice manager, assists in the development and maintenance of patient education materials in the clinic.
- Participates in the assessment of MGH medical assistants at the Salem Hospital surgery clinic as needed to maintain optimum quality assurance in all aspects of patient care. This applies to all medical assistant activities, including but not limited to sterile technique, infection control, triage issues, competencies and overall daily activities of the practice (suture removal, wound/drain care, dressing changes, patient flow, etc.).
- Ensures adherence to regulatory guidelines.
- Provides clinical training, mentoring and support for MAs as needed.
- Promotes a safety and efficient clinic space at Salem Hospital.
- Collaborates with Practice Manager on annual performance evaluations and competency assessments for the MG clinical staff.
Administrative
In collaboration with the Practice Manager and/or MGH Trauma Nurse Director:
- Participates in improvement of patient care services in a cost-effective way.
- Identifies opportunities for clinical and inpatient workflow improvement, reviews them with the Practice Manager and MGH Trauma Nurse Director and works to independently implement them.
- Represents practice on committees and performs other responsibilities throughout role.
- Completes projects on behalf of surgeons and management.
- Ability to recognize emergency situations and respond appropriately. Knowledge of locations of emergency equipment and procedures.
- Ability to assess medical history of patient, as well as patient concerns, and to determine any limitations/contraindications.
- Demonstrated skills in delivering direct, compassionate patient care at the advanced practice level, performing both collaboratively and independently.
- Advanced knowledge of human anatomy and medical terminology, including specialized clinical knowledge in the areas of Plastic and Reconstructive Surgery, Breast Surgical Oncology, Vascular Surgery, General Surgery, and Trauma Surgery. Must demonstrate appropriate knowledge of the principles of growth and development of patients of all age groups.
- Ability to question effectively to obtain pertinent medical information.
- Able to diagnose and treat health problems; obtain patient histories, perform comprehensive physical examinations, perform simple diagnostic tests / procedures, and prescribe medications.
- Advanced understanding and application of clinical care as provided in outpatient and inpatient settings.
- Able to identify and solve routine and complex clinical care matters.
- Exceptional organizational skills and ability to prioritize effectively and complete tasks; flexibility to handle multiple tasks and deadline pressures.
- Based on examination findings, the ability to recognize, identify, interpret and document normal and abnormal anatomic structures, physiology, pathology and pathophysiology.
- Maturity, self-confidence, and ability to support the management of patients across more than one surgical discipline and location.
- Highly motivated, independent, and capable of performing in a complex environment. Ability to take ownership and responsibility for activities as well as take on new responsibilities.
- Excellent interpersonal, written, and oral communication skills with good command of English language, including medical and scientific terminology. Able to effectively communicate clinical information to patients and family members.
- Ability to handle sensitive and confidential matters discreetly.
- Ability to identify issues/projects and initiate plans to address asking for direction when appropriate.
- Must work in a collaborative manner with physicians and other clinicians as well as administrative and management personnel.
- Exceptional computer skills in systems pertinent to the position.
- Knowledge and compliance with all hospital, State and Federal regulatory requirements, including hospital Policy and procedures (where applicable to the performance of the job), JCAHO and HIPAA.
- Ability to lift over 5 pounds, physically assist patients when needed. and possesses knowledge of and practices proper body mechanics.
- Knowledge of safety and universal precautions as applicable to the position and outpatient clinic setting.
- Must complete and be approved through the MGH credentialing process prior to practicing
- Some experience coordinating or overseeing the work of others required
- Current licensure as a Nurse Practitioner in the Commonwealth of Massachusetts
- Previous outpatient, ambulatory care experience in surgery or surgery sub-specialty environment needed
- Minimum of 2 years nursing experience at the advanced practice level
- Graduate of a Master’s program with a degree in nursing
