The Program Nurse Navigator will serve in a navigation role coordinating each phase of a patient s care: pre-operative, post-operative, and post-acute. The Program Navigator for Orthopedic will help develop, facilitate, grow, and manage day to day operations. Throughout their surgical stay they will be responsible for monitoring patient experience, clinical metrics using key performance indicators (data base) and bundle payments (CJR) as identified by Saint Barnabas Medical Center s leadership team. Ordering supplies for PATs/icepacks for floor/booklets/QR codes.

This includes coordinating efforts between surgeons, anesthesiologists, hospital administration, nursing, physical therapy, case management / social work and other ancillary departments while tracking the progress of the program, identifying areas requiring improvement, and making recommendations for improved patient and physician satisfaction. Lastly, this position is responsible for patient and physician satisfaction and coordinating on going community outreach efforts with Physician Account Manager.

RN Degree in Health-Related Field

Prior health education experience Preferred

Knowledge of Bundle Payments

Knowledge using Excel

Three Years of Prior Healthcare Experience Preferred

Special Equipment/Skills

Excellent organizational ability and leadership skills with the ability to work on a variety of projects simultaneously exceptional interpersonal skills and written communication skills. Basic knowledge of data collection for performance improvement activities Proficiency in Microsoft word, Microsoft outlook, power point, and excel.

Flexibility to facilitate promotional programs that may require some weekday evenings or weekends
