Nurse Navigator

Mass General Brigham is a world- wide recognized health system providing exceptional patient care within Massachusetts, New England and the rest of the globe. Brigham and Women’s Hospital and Massachusetts General Hospital are also world leaders in research aimed at improving the health status of the citizens of the world. The Mass General Brigham Sports Medicine initiative launched in 2020 with the goal of creating a regional, national, and international hub for Sports Medicine and related Specialties. Part of that initiative consists of establishing recognized centers for concussion and regenerative medicine.


The Nurse Navigator, Concussion & Regenerative Medicine will work with multiple providers in the Department of Physical Medicine and Rehabilitation to coordinate care for patients of all ages with sport injuries, relating either to head injuries/concussion or joint and muscle pain/injuries in which the patient seeks regenerative medicine treatment. The Nurse Navigator works hybrid 40 hours per week, but is expected to be on site at a MGB location that provides concussion services (including, but not limited to, Waltham, Charlestown and Foxboro) at least one day per week. This position plays a key role in triage nursing and is responsible for assuring competent, compassionate, individualized care to patients. This includes spending his/her time triaging new patient referrals, directing care to the appropriate providers, answering clinical questions by phone, directing questions to physicians as needed and supporting the clinical needs of patients. The Nurse Navigator will attend clinic at least 1 day per week. She/he collaborates with the clinical care team to achieve optimal patient outcomes. The Nurse Navigator will ensure that care is safe, efficient, effective, timely and meets the cultural, spiritual, and ethnic needs of each patient and family.

Relevant responsibilities include, but are not limited to the following:

  • Provide competent, compassionate patient-centered care based on scientific principles and the use of interdisciplinary teamwork.
  • Provide and maintain a therapeutic environment for patients and families.
  • Identify, facilitate, and evaluate outcomes of care for an individual patient or group of patients.
  • Assess, plan, implement, evaluate and modify plans of care for a specific patient assignment throughout the healthcare continuum and in collaboration with the care team.
  • Coordinate involvement of the patient, family and health team members in patient care, including patient/family teaching.
  • Document relevant patient information.
  • Provide and update educational information for the patients, families and schools as needed.
  • Communicate the plan of care and other pertinent information to/from other health care team members and patient. Promote continuity of care through effective hand-offs. This includes spending his/her time triaging new patient referrals, directing care to the appropriate providers, answering clinical questions by phone, directing questions to physicians as needed and supporting the clinical needs of the patients.
  • Educate patients regarding concussion management and return to school.
  • Educate patients regarding regenerative medicine procedure options based on their reason for visit.
  • Provide ongoing care coordination between clinic visits by communicating with patients, families, healthcare team members, school nurses, coaches, athletic trainers involved (as applicable).
  • Triage urgent/semi urgent referrals from referring physicians. This will require contacting the patient directly for more information before the appointment can be scheduled.
  • Answer medical questions by phone for current patients and families within the scope of practice as a registered nurse.
  • Provide imaging and other test results as appropriate.
  • Assist with prior authorizations for insurance approval for neuropsych testing.
  • Exhibit compliance with regulatory and quality and safety requirements.
  • Participate in daily operational activities necessary for safe patient/staff environment and continuity of care.
  • Utilize resources efficiently to facilitate optimal patient care.
  • Assist with completion of plans of prescriptions and disability or school paperwork.
  • Provide oversight, delegation and act as a resource to unlicensed staff, including medical assistants and administrative staff.
  • Provide input to the Nursing Manager.
  • Complete other duties as assigned within scope of practice.
  • Participate in professional development:
  • Maintain and update clinical knowledge and skills based on current nursing education practices.
  • Participate in and contribute to performance improvement activities.
  • Help define standards of excellence for patient care; participate in improvement of patient care services in a cost-effective way.
  • Adhere to requirements for required training.
  • Identify specific learning needs and goals and develop a plan to meet them.
Contribute to relevant committees, professional meetings, continuing educational and/or formal academic programs.

Qualifications:

  • MA RN license required. BSN strongly preferred.
  • 2-4 years of nursing experience preferred. Knowledge of sports and the pediatric and college athlete preferred.

Other Details:

  • Expected to be on site at a MGB location that provides concussion services (including, but not limited to, Waltham, Charlestown and Foxboro) at least one day per week
  • Busy ambulatory practice setting with clinic primarily out of Waltham location.
  • Hybrid 40 hours per week position.
