  • EDUCATION/LICENSE/ CERTIFICATIONS/OTHER REQUIREMENTS
      Minimum requirement:
      • Graduate from accredited program for Registered Nurses.
      • Current active licensure in the State of Kentucky as a Registered Nurse.
      • Demonstrated experience with program development/management, including data collection and quality monitoring activities.
      • Experience providing in-service education for hospital staff as well as presentations for consumers/public.
      Preferred qualifications:
      • 5 years of clinical experience in applicable service line.
      • Bachelor’s Degree in Nursing.
  • PHYSICAL DEMANDS
    • Constantly required to maintain a stationary position behind a computer or while in patient rooms and in the nursing unit.
    • Frequently required to move about while rounding on departments and patients.
    • Constantly required to communicate with people telephonically and in-person requiring talking and hearing.
    • Constantly required to operate a computer and telephone.
    • Occasionally required to lift and/or move up to 25 pounds.
    • Constantly required to lift and/or move up to 10 pounds.
    • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.

  • JOB SUMMARY
      The nurse navigator participates in the delivery of care at King’s Daughters Medical Center enhancing the patient experience through the coordination of the plan of care throughout the continuum. This position functions in various aspects of program management designed to enhance the patient experience and physician communication. The Nurse Navigator’s primary responsibility is to advocate for the patient and guide them through the healthcare system. This Nurse Navigator performs educational needs assessments and develops, implements, and evaluates educational programming to meet the learning needs of the patients and/or employees. The Nurse Navigator is committed to finding opportunities to improve the patient and family experience by integrating patient and family feedback into improvement efforts and educates the Community on applicable health issues. Nurse Navigator functions in marketing efforts to promote the growth of the applicable service line as necessary.
