Under general supervision of licensed personnel and administrative personnel the Population Health Outreach MA will schedule all routine office, hospital and non-routine in collaboration with the d locations for an assigned patient population.
In addition, the Population Health Outreach MA will perform a variety of receptionist, clerical, record keeping and miscellaneous duties in support and care of services provided in an outpatient or inpatient medical setting.
Essential Functions and Job Responsibilities:
  • Facilitates direct communication with patients discharged from hosptial and hospitalist to ensure individual plan of care is met and appointments are made.
  • Document comunication on Preventative Care Coordination note related to all patient and community outreach and coordination
  • Adheres to all WMG Compliance Program Policies and Procedures including all regulatory financial and OSHA requirements. Participates in and contributes to help achieve a total compliance program
  • Performs general office duties as assigned
  • Coordinates appoinment patient outreach, scheduling scheduling/rescheduling /canceling appointments
  • Responds to callers’ general information requests and/or inquiries; refers callers to most qualified area, if applicable.
  • Manages patient gaps in program metrics
  • Participates in quality improvement projects as appropriate
General Qualification Standards & Job Requirements:
  • Prior medical office experience
  • Excellent customer service skills
  • Ability to plan, prioritize, and complete delegated tasks in a reasonable span of time
  • Strong in both verbal and written communication

Education/Certifications/Licenses:
  • Medical Assistant certification a plus
  • High school graduate or equivalent
Performance Standards for Specialty Knowledge, Skills & Abilities:
  • Bilingual a plus
  • Strong knowledge of EMR, computer programs, and good typing skills
  • Excel Knowledge a plus
  • Excellent data entry skills
