Job Title: MAT Nurse Coordinator
Department: Medical
FLSA Status: Non-Exempt
Reports To: Integrated SUD Program Director
Hours: 4/10’s 7:30am to 6:00pm, Days TBD
Location: Center of Hope
POSITION SUMMARY:
Under limited supervision working in close conjunction with the Grant Compliance Coordinator, Integrated SUD Program Director and the Director of Nursing, the MAT Nurse Coordinator maintains the flow of the MAT Clinics and guides patients through every step of the program. They will provide outreach, support and help navigate patients that are participating in Hill Country’s MAT Plus Program. The MAT Nurse Coordinator works as part of a team to support participants in learning skills to make behavioral changes to improve their health and wellness and utilizes a harm reduction approach to promote change.
This position is based in Redding, but duties will include coordinating with MAT teams in Round Mountain as needed to include providing support as needed to care for all MAT patients. It may be necessary to work at the Round Mountain site periodically if needed as a part of this valuable role.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned which may become essential to the position.
- Verify patient’s charts to assure continuous flow of care.
- Helps rooming MA’s when needed.
- Autoclaves instruments.
- Monitors MAT Workbooks for upcoming deadlines of when assessments are coming due.
- Runs CURES reports for all new MAT patients and at least every 3 month.
- Facilitates the MAT huddles and MAT weekly meetings.
- Completes Utoxes with patient at every MAT visit.
- Do home visits as directed by patients provider.
- Ability to work independently and with others.
- Follow up of referrals.
- Completes Intake, 6 month and Discharge GPRA’s at appropriate intervals.
- Provides patient education.
- Completes MAT Agreements and TNQ with patients at intake appt.
- Schedules patients for ASAM with a Substance Use Counselor.
- Assists Grant Compliance Coordinator when needed for updating of the MAT workbooks.
- Schedules MAT patients for any non-MAT related medical issues.
- Wound care for MAT patients.
- Assists in orientation of new staff.
- Completes key grant assessments including but not limited to: AUDIT, DAST, ORT and PHQ-9 at required intervals (Intake, 6 month and Discharge).
- Assists MAT patients with any RX issues, tracks, refills and any specialty pharmacy orders.
- Performs face to face or telephone triage, consults with RN or Clinician for advice or instructions to be given.
- Will monitor MAT Nurse cell phone that patients can easily reach out to when in need of medical or pharmacy issues.
- Manages the inductions of new MAT patients on Buprenorphine. Following the COWS guidelines.
- Follows up on missed appointments to re-schedule and keep patients engaged in the program.
- Case Management.

QUALIFICATIONS:

Current licensure in California as LVN. Must have knowledge of standard nursing practices and processes of RN’s, LVN’s, MA’s and other nursing personnel.
Certificates and Licenses:
California LVN licensed. Must have current CPR certification, Blood Withdrawal Certification. Current drivers’ license and insurance.
Language Ability:
Must have intermediate level language skills. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before individuals, groups or employees of organization.

Math Ability:
Must have basic math skills. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentages.
Reasoning Ability:
Must have very high level reasoning skills. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical information and deal with several abstract and concrete variables.
Computer Skills:
To perform this job successfully, an individual should have knowledge basic working knowledge of Windows operating systems, Microsoft Office Suite - Word, Excel, Outlook, and Internet browsers. Must have working experience with medical management software such as Health Pro and Medi-Tracks.
Equipment Skills:
Must have good working knowledge of standard nursing equipment.
