Job Description

Job Summary:
The Licensed Practical Nurse (LPN) administers nursing care in a skilled nursing community under the supervision of a registered nurse (RN) or another medical supervisor and operates under the direction of physician orders. Provides basic medical care including assessing resident condition and monitoring for changes in condition, wound care, administering medication, resident/caregiver education, obtain specimens per physician orders. Ensures the health, comfort, and safety of patients by assisting with bathing, feeding, and dressing.
Essential Job Functions:

  • Acts in compliance with Salemtowne, regulatory and professional standards and guidelines.
  • Confirms Residents have received their scheduled showers and that ADL care is provided as expected.
  • Maintains ongoing communications with physicians concerning resident care including obtaining and reporting lab results, x-ray results, physical assessment abnormalities, etc.
  • Participates in and/or provide in-service education sessions.
  • Complies with evaluation, treatment, and documentation of residents within their assignment.
  • Completes required documentation in an accurate and timely manner.
  • Ensures effective interactions with patients and families.
  • Acts in the capacity as a supervisor in a modified primary care setting.
  • Ensures compliance with resident rights policies and work with families and other staff, as needed, to resolve grievances.
  • Accesses continuing education opportunities appropriate to discipline and responsibilities to maintain clinical competency.
  • Adheres to facility policies and procedures and participates in facility quality improvement and safety programs.
  • Supports Salemtowne Shines culture & philosophy.
  • Provides direct resident nursing care and supervises the Medication Aides (SNF), Medication Techs (ALF) and CNAs within their neighborhood or unit in accordance with established plans.
  • Facilitates problem solving and open communications with the nursing staff.
  • Performs other duties as assigned

  • Capable in refraining from pressuring or manipulating elders into accepting medication or treatment for the sake of convenience.
  • Capable of facilitating a sense of team connection and participation.
  • Skilled at mediating conflicts concerning team members as needed.
  • Sensitive to changing medical needs and the nature of staff responses and takes appropriate actions regardless of whether the needs have been reported by others.
  • Skilled at communicating with physicians or other medical personnel to advocate for elder preferences.
  • Able to embrace uncommon methods if they will best suit elders needs.
  • Ability to provide clear directions, expectations, and feedback.
  • Capable of eliciting trust and cooperation from team members.
  • Ability to communicate elders medical needs clearly with team members, resident representatives, and physicians.

Education and Experience:

  • Licensed Practical Nurse (LPN) license in good standing with NC Board of Nursing.
  • CPR certified or the ability to become CPR certified.
  • Graduate of an accredited Licensed Practical Nurse (LPN) educational program.

Physical Requirements:

  • Stoop and bend at the knees, twisting from side to side at the waist.
  • Capable of standing and walking short distances for extended periods of time.
  • Reach above, below, and at shoulder level.
  • Ability to lift, carry, push and/or pull objects more than 50 pounds.

Work Environment:
Works in a clean, well-lighted facility. Possible work hazards include bloodborne pathogens, moving parts, muscular pains, and strong offensive odors.
