Job Description

Duties

The PACT LPN GS-7 serves in the outpatient setting as a member of a health care team with assigned duties which include prioritization and coordination of clinical activities with minimal supervision. The employee has considerable knowledge, experience, and skill for concentrated effort and self-motivation to carry out complex patient assignments. In addition to performing the duties outlined in the LPN GS 6 Functional Description, the PACT LPN GS 7 performs a wide range of nursing care according to established policies and procedures.

The PACT LPN GS-7 is an integral part of the PACT Team, working in clinic alongside the PACT Provider, and serving in a leadership role related to the following activities:
a. Population Management (longitudinal management of a patient panel)
b. PACT Huddles (identification of complex patient needs)
c. Clinic Flow (management of care of patient during clinic visit)
d. Clinic Exit / Next Step in Care Education
e. Clinic Access Point of Contact
f. Procedures (initiate and perform routine and complex procedures)
g. Preventive Health Screening, Testing, and Consultation by Protocol
h. Education of Veterans and Family by Protocol
i. Medication and Immunization Administration by Protocol / Order
j. Customer Service (Service Level Patient Advocate and Problem Solver) k. Daily Management of LPN Staffing Assignments for the overall PACT Clinic Location

Work Schedule: Monday - Friday 8:00am - 4:30pm or 7:30am - 4:00pm. May need to stay until 5:00pm at times based upon clinic needs.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:
  • Education: Graduate of a school of practical or vocational nursing that was approved by the appropriate State agency and/or accredited by the National League for Nursing (Accrediting Commission, NLNAC) at the time of completion.
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English.
  • Licensure: Holds a current, full, active and unrestricted licensure as a licensed practical nurse or vocational nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met:
GS-07 Experience: The GS-07 grade level is for select, complex LPN/LVN positions, as established by each health care facility. To be advanced, employees must meet all of the following standards, provided the standard is part of their assignment. Completion of at least one (1) year of additional qualifying experience at the GS-06 level or equivalent, fully meeting all of the performance requirements for the GS-06 LPN/LVN. The following Knowledge, Skills, and Abilities (KSAs) must be met:
  • Skill, creativity, and initiative in contributing to the improvement of overall care delivery and patient and/or family satisfaction levels.
  • Ability to proactively assist the RN or MD/OD in addressing patient's needs for medication/treatments, accurately administering prescribed medications/treatments and appropriately recording patient's response to medications/treatments received.
  • Ability to assist RN and/or MD/DO to appropriately prioritize overall patient care needs and adjust plans for care delivery making astute observations of subtle, less obvious changes (both physical and emotional) in patient's conditions and taking the appropriate action.
  • Knowledge and skill to promptly recognize potential urgent or emergent patient care situations.
  • Knowledge and skills necessary to carry out more specialized patient care duties, including assignments in more complex inpatient and/or outpatient care settings.
  • Comprehensive practical nursing knowledge and skills in specialized procedures and practices to serve as a resource person for other staff in organizing and implementing more complex/specialized patient care.
  • Assists individuals or groups of patients and their families to take an active role in promoting health lifestyles for more positive outcomes.
  • Ability to support the RN or MD/DO in monitoring the practice of unlicensed nursing or non-medical staff in patient care delivery.

Preferred Experience: 3+ years of outpatient clinic experience and Gynecological clinic experience highly preferred.

The full performance level of this vacancy is GS-7.

Physical Requirements: The position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The position requires long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. He/she may be exposed to infected Veterans and contaminated materials and may be required to don protective clothing/equipment in isolation situations or during day-to-day clinic functions and procedures. The LPN must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on Veteran needs. The incumbent may occasionally be exposed to Veterans who are combative secondary to delirium, dementia, or psychiatric disorders. Must complete annual Employee Health requirements, such as screening or testing, as a condition of employment.

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
