Objective:
Pentec Health is hiring a Home Therapy Dietitian! The position will follow a patient on our IPN therapy through the continuum of care, this includes drafting prescriptions and educating patients, caregivers and nurses on the storage, handling and administration of IPN therapy.
Experience in home care and renal patients is preferred but not required.

Essential Duties & Responsibilities:
- Creates and provides education to the employees to keep them abreast of new information in home renal nutrition therapies.
- Participates in Quality Management meetings (i.e. Clinical Leadership, KPI, and Churn).
- Demonstrates knowledge of home therapies and operates as a subject matter expert to the sales, intake and clinical team.
- Assists with RCCM and CNCM coverage.
- Completes and supports all home therapy referral paperwork, in coordination with Intake, including but not limited to the letter of medical necessity and prescription forms for all home renal patients.
- Performs duties as assigned as an RCCM (see Renal Clinical Case Manager job description) as it relates to home renal nutrition therapy patients.
- Creates/reviews documents that will be used for customer education.
- Focuses on the education of home renal nutrition therapy patients, HHD IDPN and IPN, through the continuum of care.
- Completes the Start of Care education for home renal nutrition therapy patients.
- Follow up on home patients as deemed necessary by customer or leadership

As necessary based on business needs and at the discretion of the Supervisor.

Skills, Competencies and Experience:
Required
- Superior communication skills both verbally and written.
- Demonstrates professionalism with internal employees and external customers (referral sources, patients, and other people and/or companies as needed).
- Able to work autonomously and make independent decisions within generally accepted medical practices, regulations and guidelines.
- 2-5 years of experience in a dialysis facility with home therapies.
- Exhibits professionalism in all aspects of work.
- Experience in enteral and renal nutrition, especially in CKD 5 dialysis, along with experience in dialysis related parenteral nutrition support therapies (IDPN/IPN required).
- Works independently as needed to finish own work assignments.
- Registered Dietitian for at least two years.
Preferred
- Strongly prefer thorough knowledge of CPR+.
- Experience working remotely
- Knowledge of health insurance and third party reimbursement helpful.
Education, Certifications, Trainings:
Required
- B.S. degree in nutrition, dietetics, or related equivalent.
- Registered Dietitian or Licensed Nutritionist as required by State.
- Registered Dietitian or Licensed Nutritionist as required by State.
Preferred
- CNSC preferred or other advanced degree or certification.
Required
- Intermediate computer skills with programs such as Microsoft Office Suite: Outlook, Word, Excel and Power Point.
