JOB SUMMARY: Under the supervision of the Home Care Manager, coordinates and oversees PACE services provided in participant homes including scheduling contracted providers and employee CarePartners,.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Effectively collaborates with staff, peers, and contractors to meet PACE goals and further success.
- Acts in a manner consistent with agency values.
- Completes documentation accurately and in a timely manner.
- Consistently meets or exceeds quality assessment and performance improvement targets.
- Maintains live scheduling calendar for Home Care services.
- Receives and responds to telephone calls from Home Care vendors and CarePartners; directs to Home Care Manager as needed.
- Continually seeks better ways for delivering services and communicating with participants.
- Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence.
- Communicates effectively with participants, family members, caregivers, staff, and visitors.
- Monitors personal safety and attends to good body mechanics. Ensures infection- and exposure-control and is prepared in case of emergency.
- Complies with all policy and procedures.
- Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors, and others.
- Assists home care team in monitoring Home Care hours and duties as provided by the participant’s Plan of Care.
- Other duties as assigned.
- Demonstrates proficiency in delivering age-specific care.
- Participates in on-call coverage in conjunction with the Home Care team.
- Reports significant participant condition changes to Home Care Manager.
- Assists Center Manager and Home Care Manager in coordinating and scheduling contractor and employee trainings.
- Schedules/reschedules in-home CarePartner assignments.
- Follows agency policies for incident reports, grievances and appeals.
- Maintains master list of all participants and the current authorized in-home care hours.
EDUCATION &/or EXPERIENCE:
- One (1) year working with a frail or elderly population preferred.
- High school diploma or GED required.
LICENSES OR CERTIFICATES:
- None required.
PRE-SCREENING REQUIREMENTS:
- Pre-employment physical and PPD skin test.
- COVID-19 and Influenza (flu) vaccines preferred.
- Requires clearance of a DOJ and FBI criminal history background check
JOB SKILLS:
- Ability to work independently and with minimal supervision.
- Excellent interpersonal interactions that result in team building and contribute to effective work relationships, successful negotiation, and conflict resolution. Demonstrates effective social interaction with co-workers, management, participants, and community contacts.
- Proficiency in Microsoft Word, Excel, PowerPoint, Outlook, and relevant computer programs and software (e.g. electronic health record software). Basic knowledge of web-based applications such as timekeeping, training, and email programs.
- Ability to work in a constantly changing environment with individuals of various backgrounds and abilities.
- Ability to communicate verbally and written form in a friendly, courteous, and professional manner.
- Customer service skills necessary to work and communicate with diverse populations, and to interact and maintain relationships with program stakeholders in a friendly, courteous and professional manner.
- Highly organized and able to multi-task.
- Ability to collect data, identify pertinent information, and prepare reports and presentations.
- Maintains a positive attitude and balance in relationships with others in complex interpersonal situations.
- Maintains excellent attendance and punctuality records.
PHYSICAL REQUIREMENTS:
- Hand and arm strength sufficient to operate a keyboard for several hours each day.
- Ability to sit at a desk and interact with a computer screen for extended periods of time.
- Physical abilities sufficient to move between different staff work areas, communicate with staff and the public, operate a computer, produce reports, talk on the telephone, and travel to other Agency worksites.
