Position Overview

The Division of Developmental Disabilities is seeking to hire a Habilitation Program Coordinator for the Choate Mental Health and Developmental Center located in Anna, Illinois to serve as a Qualified Intellectual Disabilities Professional (QIDP) and a Program Coordinator/Team Leader of designated Interdisciplinary (ID) Teams. Integrates, coordinates and monitors Individual Program Plans. Provides habilitation services to assigned teams. Provides division-wide program compliance assessments.

Job Responsibilities

1. Serves as a Qualified Intellectual Disabilities Professional (QIDP)/Program Coordinator/Team Leader of designated Interdisciplinary (ID) teams.

2. Provides habilitation services for designated individuals.

3. Provides guidance and direction to ID team members in the implementation of behavioral procedures to modify individual's behavior.

4. Conducts assessments on a division-wide basis regarding compliance to accreditation/certification standards.

5. Observes individuals during daily living activities and active treatment.

6. Coordinates services both internally and externally of the Center to implement active habilitation according to the plan established by the Interdisciplinary team.

7. Conducts in-services and workshops which enhance staff services to individuals such as active habilitation.

8. Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.

Minimum Qualifications

1. Requires a bachelor’s degree in a human services professional field (e.g. sociology, special education, rehabilitation counseling, psychology).

2. A doctor of medicine or osteopathy or a registered nurse will also meet required education and training requirements.

3. Additionally requires one (1) year of experience working directly with persons with intellectual or other developmental disabilities in addition to the education and training stated above.

Preferred Qualifications (in priority order)

1. One (1) year of professional experience in applying the principles and practices for the habilitation and training of individuals with developmental disabilities.

2. One (1) year of experience communicating effectively both orally and in writing.

3. One (1) year of professional experience administering a habilitation and training program for public or private organization.

4. One (1) year of professional experience dealing with sensitive situations in relation to staff, individuals and the legal guardian of families of individuals.

5. One (1) year of professional experience directing a team in development and reviewing individual habilitation plans for a public or private organization.

6. One (1) year of professional experience coordinating services to implement active habilitation plans.

Conditions of Employment

1. Appointees are required to have successfully completed a department training program designed specifically for the Qualified Intellectual Disabilities Professional before the completion of her/his probationary period.

2. Requires the ability to physically restrain individual as necessary to prevent injury to individual or others.

3. Requires the ability to work before or after business hours, weekends and holidays.

4. Requires ability to pass the IDHS background check.

5. Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.

6. Requires ability to meet all agency vaccine/health-related policies and guidance.
