Bright Start, a program of Children’s Home Society, in Sioux Falls, SD is currently seeking a full-time Nurse Home Visitor to provide comprehensive community health services for high-risk women and children in accordance with the organization’s Mission Statement and Core Values.
What You’ll Do:
Your responsibilities as a full-time Nurse Home Visitor include:
- Consult with supervisor and peers regarding difficult clients and situations.
- Provide appropriate referrals to community resources.
- Maintain required client caseload.
- Assist with marketing and representing the program in the community.
- Develop and implement individualized care plans, prioritizing significant risk factors and considering clients’ interests and readiness to change.
- Plan and facilitate home visits with program clients in accordance with Bright Start and Nurse Family Partnership principles.
- Assess client needs status as defined in the protocol, setting measurable and achievable goals and plans.
- Communicate essential information in meetings. Provide constructive feedback to team members.
- Assure continuity of care by actively seeking participation of the client, family, support network and members of the health and social services teams.
- Complete charting and documentation of client visits and interactions.
- Obtain and record client data according to Nurse Family Partnership model and Agency protocol.

Schedule:
- This is a remote work through home visitations while officing from home. Company vehicle provided.
Monday-Friday daytime hours, but flexible schedule is required. 40 hrs/week.
This position primarily serves the counties of McCook, Lincoln, Turner, and Minnehaha
Compensation: $26.60/hr+ applicable education and work experience

Experience/Education/Skills:
Bachelors degree in nursing preferred. Valid Registered Nurse License in the State of South Dakota. Previous experience in women’s health care, pediatrics, home visitation services, assessments and case management preferred. Must possess valid South Dakota driver’s license.
Physical Demands and Work Environment:
Regularly in normal office environment. Area travel and client home visits regularly required. This position often requires lifting infants and children. The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 100 pounds. Long and irregular hours required from time to time.
