Director Of Care Management (Nursing)

Job Summary
The Director of Care Management will be responsible for:

• Building and maintaining connections with key community and business partners such as hospitals, post-acute providers, community resources, state agencies, payers, and purchasers of care.
• Sculpting a vision of Care Management with the Patient Care Services leadership team at BCH that is positioned to improve the quality and reduce the cost of care..
• Mobilizing and inspiring a cross-functional team to see this vision through.
• Developing related programs and processes that meet current strategic needs and anticipate future imperatives.
• Coordinating with finance and contracting leadership in matters relating to payment models, public program contracts, and applicable federal and state regulatory mandates.
• Creating a coherent and unified system that ensures all patients have access to the resources they need to navigate a complex landscape of partners and decisions along the continuum of care.
• Building trusting relationships with key stakeholders at BCH and beyond, based on mutual intent.
• Designing the right forums for and methods of capturing best practices and relevant institutional knowledge from across BCH as it relates to coordinating care.
• Collaborating with leaders across the institution to ensure these efforts build from a strong foundation of research and evidence-based activity.
• Overseeing large-scale, complex, cross-organizational projects.
• Ensuring adherence to all regulatory expectations.
• Helping to sculpt and deliver the next generation of care management at the hospital and across a diverse spectrum of partners.

To qualify, you must have:

• Excellent leadership and teambuilding skills.
• 10+ years of team management and operations within large, complex, multi-site healthcare organizations.
• Graduated from an accredited program of Nursing
• Strong critical thinking skills, and ability to create and implement high level strategy.
• Exceptional interpersonal and communications skills.
• Excellent working knowledge of coordinated care solutions and operations required
