Position Summary

Under direction of the ACT/CPT Program Manager and within federal, state and local guidelines provides moderately complex case management work. Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. The CPT QMHP acts as a community liaison to connect individuals within Denton County with services through MHMR or other services covered through their insurance as needed. CPT QMHP often coordinates with different police departments regarding individuals who may be in a crisis or need support. The CPT QMHP coordinates the Familiar Face Task Force (FFTF) as well, which is composed of individuals within the community and individuals already in services through MHMR, who have had multiple crisis episodes, interactions with crisis teams or CIRT. The CPT QMHP also conducts weekly outreach with The Junction to provide resources to individuals in need of resources or information on how to access MHMR services. Within the center, the CPT QMHP also provides extra support to clients aside from their assigned case manager in order to increase engagement and reduce crisis.


Essential Duties
  • Monitors a case load of high utilizers and visits them as needed to increase engagement; provides skills training, engagement activities, transportation, or other assistance to clients as needed to help them connect to outpatient services; communicates effectively with case manager and other providers about progress and barriers; completes monitoring reports as determined by the Program Manager.

  • Keeps daily service activity logs on all direct care activities provided; enters time worked and submits time sheets.

  • Contact supervisor or designated back-up for clarifications of procedures or clinical assistance as needed; communicates urgent clinical and logistical needs with appropriate manager.
  • Focuses on the Center’s goal of reducing hospitalizations; leads the monthly Familiar Face Task Force (FFTF) meeting and follows up on any cases to ensure appropriate follow through occurs; keeps an active list of high utilizers of hospital bed days; keeps the FFTF accountable for discussions that occur in the meeting; completes meeting minutes and sends them to all participants and Medical Director.

  • Works effectively with local law enforcement, EMS, and community agencies using the CPT model; participates in weekly community outreach efforts using the CPT model; participates in ride alongs with law enforcement to mental health calls as per request of law enforcement agencies.

  • Provides thorough documentation; documents all services provided to a client or on behalf of a client in a progress note; each service delivered will be documented according to agency and stated guidelines to also include any clinically significant information and in the case of a crisis and/or emergency; documents all skills training groups for each assigned client in the client’s medical record chart; documentation to be completed as services occur but no later than 2 business days.

  • Attends all scheduled clinical team meetings; meets with supervisor for monthly individual supervisions.

  • Act as back up to MCOT and carry out tasks assigned which may include client care as well as office duties; completes all tasks assigned including but not limited to ROH Assessments, follow up appointments, case management appointments, phone calls, aftercares, and transports.

Education and or Experience

Graduation from an accredited four-year college or university with major course work in a human services field as required to qualify as a Qualified Mental Health Professional (QMHP) and Three (3) to five (5) years prior experience in a mental health field required. Master’s Degree in a human services field and previous Crisis experience preferred. English/Spanish bilingual preferred.


Registration, Certification, Licensure & Qualifications
  • All job specific training must be completed within 30 days of hire.
  • Must maintain a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.
  • Must qualify as a QMHP (Qualified Mental Health Professional).
