PURPOSE OF POSITION:

Under supervision, aids management executives in operating a health care facility and assists in supervising delivery of a group of services. Performs administrative and managerial functions related to the operation of total facility or specific patient services. Implements and coordinates new programs and projects and monitors their operations and effectiveness. Manager is responsible for the administrative functions of the clinics and support listed below.

  • Integration of IMPACT {Integrated Model for Parents and Children Together) services within Primary Care Pediatrics and OBGYN
  • Healthy Steps
  • Reach and Read Coordinator
  • Video Intervention Project
  • Care Coordination
  • Supervise Support Staff
  • Social Work/Mental Health care coordination

SUMMARY OF DUTIES AND RESPONSIBILITIES:

Project Management Activities:

  • Responsible for day-to-day operations for services supported by the IMPACT project within Pediatrics, Behavioral Health, and OBGYN (VIP, Healthy Steps, Reach Out and Read, Social Work, Mental health and Care Coordination).
  • Coordinate activities with community partners, scheduling meetings, drafting minutes, ordering supplies, and other administrative tasks as required by the project.
  • Task supervision for Community Health Worker
  • Support staff to create, implement and consolidate work flows.
  • Accountable for and manages development, implementation and ongoing maintenance of facility-based projects in support of IMPACT.
  • Coordinate Reach Out and Read (ROR) program (recruit and supervise volunteer readers, secure sustainability funding, book purchase, book distribution to providers, manage funds/ bookkeeping and quarterly submission of program data to national office and Greater NY Coalition office).

Referral Management Activities:

  • Maintains database of patients screened and referred, including getting monthly data for performance improvement on screening rates, positive rates and responses to positives.
  • Establishes and maintains positive relationships with referral sources both internal and external to NYC H+H, ensuring patients can obtain referral appointments with established referral providers, as needed.
  • Ensures adherence to standard work policies and procedures regarding referral processes, including troubleshooting concerns by referral providers and conducting regular communication on referral workflows/requirements for compliance.

Minimum Qualifications


1. A Master’s degree from an accredited college or university in Public Health, Public Administration, Business Administration, Social Work, Psychology or Rehabilitation Counseling; and
2. Two (2) years of full-time experience in medically-oriented health care and medical support systems environment, one (1) year of which included experience in an administrative or supervisory capacity in such occupations as physical, occupational, inhalation, speech and audiology therapy or as Medical/Laboratory Technologist, Microbiologist (various specialties), Chemist (biochemist), Radiation and X-Ray Technician; or two (2) years of responsible level experience in business management systems or general administration including one (1) year in a supervisory or administrative capacity; or
3. A Baccalaureate degree from an accredited college or university in disciplines listed in “1” above; and
4. Three (3) years of full-time experience, as described in “2” above, including two (2) years of experience in an administrative or supervisory capacity in occupations, as listed in “2” above; or
5. A satisfactory equivalent combination of education, training and/or experience.
Note: Certification or licensure may be required.

QUALIFICATIONS FOR THE JOB:

  • Competence in computerized systems related to electronic medical records, scheduling and analytic systems.
  • Strong organizational and time management skills.
  • Demonstrates effective interpersonal, verbal and written communication skills.
  • Strong analytical problem-solving skills.


Computer skills, including Microsoft Office, high level in Excel and databases. Meeting community health needs, health care planning, financial planning, operations.
