Job Summary
40-hour Social Worker needed for the Boston Children’s Hospital Primary Care Center. Boston Children's Primary Care at Longwood helps families raise healthy, happy children. Located on the main campus of Boston Children's Hospital, we serve more children than any other primary care practice in Boston. Our services include wellness checkups and preventive care, immunizations, same-day visits for illnesses and other concerns, services and support for children with special health care needs and mental health and social support. LICSW supervision provided.
This Primary Care Clinical Social Worker will be responsible for:

• Performing psychosocial and risk assessments, assigning diagnoses and developing and implementing interventions and treatment plans.
• Participating in planning and implementation of discharge and outpatient service plans and collaborating with health care team and community agencies to plan
• Implementing and following up on treatment and provision of needed services.
• Providing clinical social work services to assist patients and families with personal, family, medical and environmental difficulties.

To qualify, you must have:

• Experience with social services agencies, understanding of social determinants of health and trauma ideal.
• Master’s Degree in Social Work, and LCSW is required. LICSW is preferred
• Bilingual/bicultural/diverse candidates are encouraged to apply. Spanish speaking a plus!
• Flexibility and ability to work in fast-paced multidisciplinary team environment as well as excellent communication skills.
