Unique Ambulatory Specialty Pharmacy Position
The clinic specialty pharmacist is responsible for the operation and services provided within the specialty medical practice or remotely through centralized telephonic clinical support. Delivers a high level of service to each patient in every interaction, ensuring they perceive Baptist Health’s pharmacy as the pharmacy of choice and ultimately impacting their level of success in their therapy. Provides clinical consultation, chronic disease state management, medication reconciliation, and adherence counseling for specialty patients. Plays an integral role on the interdisciplinary team including pharmacists, billing specialists, nurses, medical assistants, and providers. Facilitates the development, implementation, and maintenance of programs that promote desirable patient outcomes through safe, appropriate, cost-effective medication therapies. Ensures compliance with pharmacy regulatory standards.
- Maintain pharmacy onsite or centralized clinic services, operational policies and procedures, clinical pharmacy workflows and practice expectations.
- Partner with providers and clinical staff on integration of the pharmacy team to coordinate care with eligible patients.
- Educate clinic and retail pharmacy teams on specialty pharmacy service offerings and demonstrate the value to the practice and system.
- Communicate with physicians and other clinicians as necessary to facilitate exemplary service for new and existing clinic patients.
- Enroll specialty patients into the program by assisting with recruitment of patients to ensure the best possible clinical outcomes.
- Communicate with patients and patient families about the benefits of filling their prescriptions with Baptist Health.
- Provide initial clinical medication counseling and regular patient reassessments to ensure appropriateness of specialty therapy.
- Perform medication therapy and disease state management, which may include coordinating medication therapy adjustments to optimize patient medication regimens and improve clinical outcomes.
- Provide safe, timely, efficient, equitable, effective, and patient-centered pharmacy patient care.
- Refer appropriate patients to financial counseling for copay assistance through manufacturer or charity programs.
Education/Experience
- Experience in ambulatory clinical pharmacy practice preferred.
- Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required.
- Kentucky Board of Pharmacy license in good standing.
- Possesses knowledge in drug information, third party claim adjudication and regulatory requirements of pharmacy practices.
- 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.
- Disease state specialty experience preferred.
