Position Summary
This is a professional position that provides services in the field/ home/ community. The Mental Health Court Care Coordinator serves those adults identified by the Pinellas County Court for the Mental Health Court Diversion Program. The intent is to assist individuals who have entered the criminal justice system, due to unmet mental health needs, transition successfully to effective community based care. The Mental Health Care Coordinator will enhance the delivery of treatment services and recovery supports to the priority population. This includes services and supports that affect a person's over all wellbeing, such as primary physical health, and social connectedness. The Care Coordinator will connect systems including behavioral health, primary care, peer and natural supports, housing, education, vocation and the justice system. The Mental Health Care Coordinator is dedicated to assisting individuals living with a mental health condition to remain living within their community with the services and supports they need.
Responsibilities include collaborative coordination between the individual, the courts and the services. Additionally, the Mental Health Care Coordinator is responsible for the development of a culturally specific individualized services plan in partnership with the individual and court which reflects the individual's strengths and self - identified goals. As an integral part of Mental Health Care Coordination services, discharge/transition services are arranged or coordinated by the Mental Health Care Coordinator as the individual is preparing for discharge and/or transition from one level of care to another. These services are designed to support the attainment of individual-defined goals as identified in the individuated Treatment Plan (e.g. stable living arrangements, quality relationships, employment, vocational training and/or stable mental and physical health.) All services will be provided within a recovery-oriented system of care, which builds on the individual's strengths and incorporates community, natural systems, and clinical supports and services in the management of the individual's needs.
Education Experience/Requirements
  • Proficiency in completing progress notes, treatment plans, and pertinent information concerning the consumer required.
  • Bachelor's degree in Human Services from an accredited college with a minimum of one year of previous experience working with adults and children with serious persistent mental illness.
  • Completion of Behavioral Health Case Manager Certification within 90 days of hire required.
  • Must have excellent writing, communication, computer, organizational and time management skills with the ability to work independently and as part of a team.
