Quartz is seeking a Care Coordinator - Clinical Programs to join our team. This position provides care coordination services to meet promoting health needs (medical, behavioral health, and social) of Quartz members while promoting and maintaining the quality of care for members in partnership with leadership, sales, and care management.

  • Coordinate and monitor wellness events for multiple large group clients within software scheduling tool.
  • Provide referrals for community resources and clinical programs.
  • Collaborate with physicians, social service providers, and others by involving them in the care coordination process.
  • Actively monitors member’s ability and willingness to engage in treatment regimes, and problem solve with staff, members, and providers, offer alternatives or solutions to their needs.
  • Promote Quartz, Provider Owners, Employer groups, and community-based programs and offerings to members through outreach.
  • Assist in problem resolution involving members and providers as a member advocate including information for the member grievance process.
  • Provide opportunities for in-person, face-to-face meetings with members in their home or community as needed.
  • Maintain a dashboard of existing groups’ promoting health events and publish weekly.
  • Document member information and contacts made regarding member’s care, services provided and social determinants of health.

 

  • Associate degree in health education, psychology, counseling, social work, or related field with 6+ years of experience in healthcare or social work
  • Proficiency in speaking Spanish, Hmong, or other commonly used foreign language is preferred
  • Preferred experience:
    • Working with Electronic Medical Records (EMRs)
    • Related health care or social work experience
    • Experience with Motivational Interviewing and trauma informed care
  • Experience with Motivational Interviewing and trauma informed care
  • Ability to effectively communicate thoughts, ideas, and information both orally and in writing
  • CSW (Certified Social Work) is preferred
  • Able to work independently in Microsoft Office applications with ability to learn documentation systems such as Epic and Altruista
  • Knowledge of local, state, and federal community resources available to members
  • Strong interpersonal skills with ability to effectively interact with members and coworkers from a variety of different background and experiences in a professional and courteous manner
  • OR Bachelor’s degree in health education, psychology, counseling, social work, or related field with 3+ years of experience in healthcare or social work
  • Ability to demonstrate flexibility, set priorities with daily demands and long-term work assignments and projects
  • Able to problem solve and provide solutions using appropriate methodologies
  • Working with Electronic Medical Records (EMRs)
  • Related health care or social work experience
  • Knowledge of Medicare and Medicaid benefits

This position can work 100% remotely or from our office in Madison, WI in a hybrid or full-time on-site arrangement, if preferred. Hardware and equipment will be provided by the company, but candidates must have access to high-speed, non-satellite Internet to successfully work from home.
