The Behavioral Health Case Manager is part of a community-based crisis team that provides alternative response with the goals of providing on-site community-based intervention such as outreach, de-escalation, stabilization, resource connection, and follow-up support to people experiencing behavioral health crisis. As part of a multidisciplinary team, the Behavioral Health Case Manager will provide telephonic and/or in-person follow-up, engage in community outreach, coordinate, and collaborate with other internal/external service(s) and support(s), and assist with transition to long-term services/supports.
Key Duties
  • Collaborate with community providers, emergency rooms, evaluation and treatment facilities, and housing programs to provide holistic, person-centered care and coordination of wraparound services.
  • Offer liaison support to people seen on a recurring basis that require short-term intensive case management.
  • Coordinate problem-solving and engagement activities among the team with similar and different specialties and skillsets.
  • Participate in the care coordination and collaboration with members of the team to develop services and supports to include safety planning, treatment recommendations, connection to housing and stabilization, and transportation to voluntary services.
  • Communicate on a regular and timely basis with supervisors, other staff, people receiving services/supports, their families, community supports, and with professionals outside the agency.
  • Develop and maintain working relationship with all community partners.
  • Facilitate the transition to on-going behavioral health/substance use disorder services by providing referral(s) to people who have had contact with the Behavioral Health Crisis Response Team. This will include completion of warm handoffs when necessary.
Physical Requirements & Working Conditions:
  • This position may drive a mobile unit vehicle or other vehicle (that is provided by the city).
  • Incumbents in this position may be subjected to hazardous chemicals, adverse weather conditions, communicable diseases and possible verbal and physical abuse from hostile and disorientated persons, as well as those who may be under the influence of illicit drugs or have an undiagnosed/diagnosed behavioral health condition.
  • Ability to work days, evenings, weekends, and holidays. Specific schedule will be determined with direct supervisor.
  • Requires manual dexterity, balance, eyesight, and hearing.
  • Working conditions include both an indoor and outdoor environment.

Qualifications
Minimum Education*
Bachelor's degree in social welfare, psychology, social work related field.

State of Washington Driver's License
