Associate Program Manager, Community Health

Assists with developing strategic community and population health programs that measurably improve the health and wellbeing of the communities served. Implements program design plans requiring assessment, research, and analysis. Assists with coordination, engagement and collaboration of community partners and internal stakeholders to align community needs and business imperatives and identify and deploy resources that support community health improvement. Supports the implementation of program plans and evaluates outcomes.

Job Responsibility

• Prepares additional recommendations and adjustments to plan, as needed.

• Evaluates analyses and identifies high risk populations and makes strategic program recommendations; conducts research; identifies industry standards and new innovative practices for analysis.
• Implements risk-based programs; partners with the leadership to implement program initiatives; monitors program plan performance and prepares status reports.
• Operates with direct guidance and work assignments are generally straightforward and of moderate complexity.
• Collects data for review and analysis.
• Assists in preparing operational analysis, flow charts, presentations and reports, as required by program specifications; presents analysis and recommendations to management; obtains feedback and collaborates with management to develop program improvement plans.
• Assists in preparing program assessment, analysis, and strategy recommendations; interfaces with management to collect statistical, utilization, financial, quality and risk performance data for analysis.
• Prepares analysis and reports for management.

Preferred Skills

• Proficiency in speaking Spanish is desirable, but not required.
• Strong project management, research and interpersonal skills, experience with grant writing and administration, content expertise in addressing health related social needs, including, but not limited to food and nutrition insecurity, mental health, maternal health and school-based/youth programming is highly desirable.

Job Qualification

• Bachelor's Degree required, or equivalent combination of education and related experience.
• 0-1 years of relevant experience, required.
