NewYork-Presbyterian has a long tradition of bringing hope, health and healing to our communities. NYP is continuously implementing new programs to meet the unique needs of our communities, and strengthening those programs that exist. Our Division of Community and Population Health programs build upon, and tie together, the Hospital’s inpatient care coordination, Ambulatory Care Network, community outreach programs, and the effort of community-based providers.

Started in 2005, the Center for Community Health Navigation (CCHN) at NewYork-Presbyterian Hospital is dedicated to support the health and well-being of patients through the delivery of culturally-competent, peer-based support in the emergency department, inpatient, outpatient, and community settings with the ultimate goals to promote healthcare self-management and to decrease avoidable system utilization. In 2018, NYP received a generous gift to expand these efforts to NYP Brooklyn Methodist Hospital, NYP Queens, and NYP Lower Manhattan Hospital, collectively forming the Robert Center for Community Health Navigation.

Program Manager – Emergency Department Patient Navigator Program - Center for Community Health Navigation

Step into this pivotal onsite role, where you will be responsible to develop and lead the ED Patient Navigator Program and Inpatient Patient Navigator Program across three emergency departments and inpatient units at Columbia. The Program Manager will work alongside health care teams in the emergency department, inpatient, and outpatient settings and with community leaders to develop these programs designed to support patients who are not well established with care and who need additional support and resources to manage those social determinants of health that interfere with their overall health and wellness.

Preferred Criteria

• Excellent organizational and analytical skills and a high degree of flexibility
• Strong written and oral communication skills
• Bilingual Spanish
• Master’s Degree in Public Health Administration or equivalent experience

Required Criteria

• Experience working in a community setting
• Ability to function independently with minimal supervision
• Experience in program development, implementation, and evaluation
• Minimum of 3 years of experience in a supervisor or manager role
• Bachelor's Degree

Salary Range

$80,500-$115,500/Annual
