We are currently looking for an Assistant Manager Hospice to join our team. This position will be responsible for providing coordination of internal and external services for patients on the Hospice team.


Responsibilities


  • Monitors quality and appropriateness of services provided to ensure patient satisfaction
  • Triages phone calls and schedules visits on a daily basis
  • Makes home/facility visits as needed and directed by the Clinical Manager
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Collaborates with physicians and other health care providers regarding client’s needs
  • Provides education and serves as a resource for field staff
  • Covers for the Clinical Manager as directed by the Clinical Manager/CNO
  • Provides orientation, supervision, clinical oversight, and support to branch staff as necessary
  • Adheres to agency infection control and safety policies


Qualifications


  • Knowledge of reimbursement and HIS requirements
  • Excellent verbal and written communication skills
  • RN, BSN preferred
  • Strong organizational and interpersonal skills
  • Valid driver’s license
  • 2+ years of community health experience with 1+ year of prior experience with the agency


Working Conditions/Physical Demand : Working indoors and outdoors with phone and computer use; must be able to lift fifty pounds (50 lbs.)
