Minimum 1 year project management/budget experience

Pay: $58,937 - $68,495/year , DOE (+differential for bilingual skills). Benefits available

Provide oversight and management of the Camp HOPE America – Oregon & Pathways

Provide oversight and management of Youth Advocacy services; assist with community

Represents CWS at community meetings, speaking engagements and educational

Provides case-management services within the Youth Services team for probation involved youth and their families.

Manages a fast-paced workday and can meet productivity requirements for program.

Understands the difference between Clinical and Case Manager role. Case Manager seeks consultation regularly and acts within scope of role.

Experience with youth and families strongly preferred.

About the role of the Case Manager:

Provide ongoing case management, crisis intervention, conflict resolution, and stabilization services to youth enrolled in the program.

Maintain accurate documentation of client's goals and needs, case management plans, and service entries into internal data systems.

Enter program data and outcomes into the region's Homeless Management Information System (HMIS) to adhere to grant and contract compliance.

Document thorough case notes that accurately reflect client progress toward individualized case management goals.

Education and/or Experience: 1-2 years of experience working with people experiencing homelessness, foster care, and juvenile dependency systems.

Other Qualifications: Experience managing crises and implementing crisis interventions.

Excellent demonstrated leadership and management skills, particularly with an ability to influence and motivate others

Strong organizations skills ability to manage multiple projects, deadlines and competing priorities;

Knowledge and experience working with young adults experiencing homeless and other at-risk youth;

Knowledge of systems impacting young adults, including benefit system, school systems, employment, health and mental health

Follow up and provide support to Case Managers on any incorrect requisitions

Proficient with Windows, Outlook, internet research, MS Office including Word and Excel. Experience with proprietary software preferred

Contacts parents/legal guardians and schedules appointment with clinical specialist; initiates referrals to needed community resources when appropriate, including follow up contact.

Handles telephone calls to provide assistance, facilitates entrance; provides information regarding community resources.

Interfaces with law enforcement and other referring parties to obtain pertinent information.

Screens and assesses youth's behavior as well as physical and mental health to determine appropriate services.

Checks youth belongings for weapons or contraband and confiscates unsafe or illegal materials.

Completes case recording documentation and all pertinent information in individual client's records as per agency policy.

Manage and monitor client finances within required time frames.

Minimum of 2 years of experience providing direct support/care to clients in a Mental Health/ Intellectual Disabilities setting.

Minimum of one year of leadership and/or supervisory experience.

Bachelor’s Degree OR 8-10 years patient services/case management experience.

Assess benefit plans for coverage and limitations and documents necessary information for provider education.

Demonstrates effective management of CRM system while documenting pertinent information as required by standard operating procedures.

Will interpret benefit verifications including PA requirements and drug acquisition options for specific patients enrolled in GPS.

Provides coverage and benefits on specialty pharmacy and buy & bill acquisition options.

High comprehension of scientific knowledge and the ability to communicate with patients in a compliant and compassionate manner.
