Qualifications (Education, Training and Experiences)

High School graduate or equivalent required. College education in Mental Health or related field preferred.

Experience in working with children or adolescents required (minimum 1 year)

All other duties assigned by direct supervisor or KidsPeace Leadership.

Fluency in English and Spanish (demonstrated competency in oral and written communication) required.

3 to 5 years Case Management experience preferred.

Provides medication management, including regular medication reconciliation and support of medication adherence;

Identifies problems/barriers for care coordination and appropriate care management interventions.

Completes comprehensive person centered assessment inclusive of physical health history, mental health history, social determinants of health and supportive needs.

Coordinates physical, behavioral health and social services;

One year of youth experience. Or a combination of education and experience related to working with youth.

Bilingual Spanish abilities highly preferred (verbal communication skills)

Basic computer skills using Microsoft Word, Excel, databases and Outlook
