Overview A Remote Case Manager is a professional who works remotely to provide case management services to clients. They are responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating services for clients in order to ensure that their needs are met. They also provide support and guidance to clients and their families. Detailed Job Description A Remote Case Manager is responsible for providing case management services to clients. They must assess the client’s needs and develop a plan of care that meets those needs. They must coordinate services with other providers, monitor the client’s progress, and evaluate the effectiveness of the services provided. They must also provide support and guidance to the client and their family. They must be able to work independently and be able to communicate effectively with clients and other providers. • Knowledge of case management principles and practices

Job Qualifications
• Bachelor’s degree in social work, psychology, or a related field
• Master’s degree in social work, psychology, or a related field preferred
• Licensure or certification in case management preferred
• Previous experience in case management preferred

• Knowledge of community resources
• Knowledge of assessment and planning techniques
