Previous medical surgical nursing experience

American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification

American Red Cross or American Heart Association Pediatric Advanced Life Support (PALS) Course and Certification within 90-days of hire

Three (3) or more years hospital/acute care experience. Emergency medicine experience preferred

Proven clinical & leadership experience preferred.

Associates Degree required (if enrolled in BSN program)

Detail oriented and highly organized with effective time management skills.

2 years of Behavioral Health experience

2 years of Med Surge experience

Clinical quality experience is a plus

Minimum three to five years clinical and/or medical management experience

Provide members choices in medical care providers and services based on the members’ clinical needs, geographic locations and available benefit offerings.

Connect with customers by phone and quickly develop a rapport to help the individual to navigate their benefit choices and options.

Research providers through credentialing, education checks and affiliations with notable medical centers.

Strong Communication skills and phone etiquette

Strong problem solving/issues resolution skills

Degree in nursing RN (Required) with at least 2 years experience

Experience working with chronically ill (CKD/ESRD) patients

80% On location Travel for Patient Home Visits and (Practice Office)

Participating with Care Team and physicians to prevent readmission, prevent future hospitalization and improve patient care

Become proficient in learning the Aerial program to review information re: Case Management

Attends weekly Health Partners meeting with Benefit Team at Whirlpool, Account Managers and Health Coaches

Demonstrated team leadership experience and/or skills

Clinical experience and/ or a Managed Care background

Knowledge of medical claims administration and employee benefit plans

Attend meetings with account manager, client, vendors, and other healthcare team members as needed

Conducts member health coaching and provides treatment decision support service via printed educational materials and videos

5 years recent experience in the acute care setting preferred

Excellent written and verbal communication skills

Outstanding team player & strong interpersonal skills

Strong organizational skills and the ability to multitask with ease

Serve as the ongoing coordinator of primary and preventative care, episodic care, expert medical opinion and other service lines

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

Preferred Experience: (Not with the documents. Please provide preference)

The RN Patient Care Manager will support the mission of the Company and the Care Management department.

Care Manager documents assessment and interventions in real time in the electronic medical record.

Education: A graduate of an accredited RN Nursing program required.

Previous Experience: Minimum of two (2) years of oncology experience required. Minimum of 5 years of overall nursing experience required.

Office experience: Prior experience with customer service and professional phone etiquette required.

Certifications/Licenses: Active RN license in state of employment. OCN preferred

Two years nursing experience preferred. Current CPR certification required.

Greets all patients with a warm and welcoming tone

Demonstrates effective communication using AIDET techniques on every call

Verify patient demographic information and accurately enter same into computer system.

Assist patients in understanding account balances

Be familiar with policies of various insurance companies.

Participates and facilitates in the case management process and cost containment for patients.

Strongly prefer candidates to have at least 1 year of RN experience in acute care

Must have BLS, ACLS and PALS certifications.

Cooperates with other personnel to achieve department objectives and maintain good employee relations with collaborative integration of the health care team.

Demonstrates accountability for direct patient care and evaluation of patient outcomes

Candidate must have successfully completed an accredited RN program.
