Have working knowledge of all residents under his/her care.

Knowledge of procedures and techniques necessary to administer medications and treatment as prescribed by physician.

LPN - Skilled Nursing - FT Night Shift - Up to $29/HR

Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent life-style of residents.

Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.

Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.

On each assignment, provides skilled nursing care/services in accordance with prescribed orders

Identifies and prioritizes actions based upon patient/client care requirements and/or contracted customer requirements

Attends case conferences, team meetings and education in-services, as required

One year prior professional nursing experience preferred

Willingness to participate in skills advancement training or novice nurse program if applicable

Must meet all federal, state, local, and contract requirements

Plans and manages patient care according to each patient’s needs

1+ years of clinical experience as an LPN

Any experience working with elderly patients or veterans is preferred

$40 per hour pay rate

Concentrate and use reasoning skills and good judgment.

One year nursing experience preferred.

CPR Certification. CPR certification must remain current during employment.

Implement and interpret the programs, goals, objectives, policies, and procedures of the department.

Exhibit excellent customer service and a positive attitude towards patients.

At least one year of experience in an Assisted Living/Memory Support facility

At least one year of experience at this level of responsibility

Excellent verbal and written skills

Perform resident assessments as required by regulations and facility policy. Participate in evaluation of new residents and level of care transitions.

Assist with departmental planning and budgets. Arrange for appropriate supplies and equipment, maintaining compliance with departmental budget.

Perform other duties as assigned. Attend in-service classes and staff meetings. Assist in developing/updating written policies and procedures and job descriptions.

Knowledge of documentation requirements related to timeliness, accuracy and completeness

Performs other nursing services and responsibilities as assigned by the Resident Care Director or Administrator.

Good planning, initiation, delivery and evaluation skills with regard to nursing care

Working knowledge of State regulations as they apply to the assisted living setting

Knowledge, Skills, Abilities and Competencies

Performs nursing procedures or specialized functions in accordance with knowledge, skills, and licensure.

Participates in the evaluation of responses to interventions.

Assists in assessing, reporting, and recording health status of patients, families, and groups in a variety of settings.

Participates in developing and implementing appropriate aspects of a plan of care.

Reports appropriate patient information to supervisor or appropriate health team member.

Six months practical nursing (LPN) experience

Completion of an accredited training program for Practical Nursing

Licensed as a Practical Nurse by the State of Minnesota

Current LVN or LPN License with CPR Certification

Manages conflict in a positive and constructive manner

Ability to work independently with advanced critical thinking skills
