Provide initial triage screening and assessments, risk assessment, crisis management, psychoeducation, and referrals to campus or community services as appropriate.

Three (3) years of professional experience in Student Affairs, Higher Education, Counseling, Social Work, or a closely related field.

Doctoral degree in relevant mental health field (Social Work, Counseling, Clinical Psychology, Human Services Field, or Higher Education/Student Personnel)

Experience providing services to a college population.

Experience providing crisis, triage, and/or intake mental health services.

Three (3) years of professional experience in a supervisory or leadership capacity.

Knowledge of insurance and managed care practices preferred.

Provides education and customer service to providers, staff and patients regarding the department referral process.

Provides data entry and clerical support for the referral process for both pre-paid and fee-for-service health plans.

Understands each health plan’s guidelines, benefits and basic risk models.

Takes on special responsibilities and projects in areas as requested.

Associate’s degree in business or healthcare, or equivalent experience, required.

Manages systems, processes, staffing, budget, and outcomes.

Utilizes the interdisciplinary model to promote cost effective quality care within the hospice environment.

Practices in accordance with the mission, philosophy, policies and ethics of Alive Hospice, Inc., and NHPCO guidelines.

Serves as liaison, educator, and resource for hospice in the professional and community arena.

Aides in achieving agency goals through support, project work and team building.

Provides clinical and administrative supervision to staff including recruiting, selecting, training, and counseling employees, staff assignment and scheduling.

Preferred Skills, Capabilities and Experiences:

Initiates and manages clinical referrals for participants registered in a disease management or health program.

Prior Experience working in Social Service office, Behavioral Health groups, etc. preferred.

Knowledge of Microsoft Excel highly preferred.

Ability to self manage multiple data streams preferred.

Collaborates with the Field Staff Scheduler, Manager or designee to identify interdisciplinary team staff, if available, to facilitate patient admissions.

A minimum of two (2) years of telephone-related experience required; experience within a health care environment highly preferred.

Processes all referrals coming into the organization via phone, fax, email and extended care information network (ECIN).

Proactively responds to crisis calls, taking appropriate action to resolve the situation for a positive outcome.

Collaborates with the Field Staff Scheduler, Manager or designee to identify interdisciplinary team staff, if available, to facilitate patient admissions.

A minimum of two (2) years of telephone-related experience required; experience within a health care environment highly preferred.
