Description


Coordinates daily operations of Intake and Admissions processes and has administrative responsibilities for care and coordination of Intake and Admissions staff.


Essential Duties And Responsibilities


  • Ensures Alive Admission visit is consistently performed by each Admission nurse.
  • Collects and analyzes Referral, Intake and Admission data to identify trends and opportunities for improvement.
  • Promotes excellence in nursing practice.
  • Utilizes the interdisciplinary model to promote cost effective quality care within the hospice environment.
  • Collaborates with staff of other departments as appropriate.
  • Other duties may also be assigned.
  • Monitors customer service-related activities to optimize performance.
  • Practices in accordance with the mission, philosophy, policies and ethics of Alive Hospice, Inc., and NHPCO guidelines.
  • Provides clinical and administrative supervision to staff including recruiting, selecting, training, and counseling employees, staff assignment and scheduling.
  • Aides in achieving agency goals through support, project work and team building.
  • Works collaboratively with the Vice President of Quality and Compliance to ensure smooth transitions from referral to admission to ensure continual compliance.
  • Supervises and allocates clinical staff based on agency needs.
  • Coordinates admissions to Alive Hospice in all areas of delivery of patient care.
  • Serves as clinical leader as designated and is in the full rotation of Director on Call
  • Serves as liaison, educator, and resource for hospice in the professional and community arena.
  • Manages systems, processes, staffing, budget, and outcomes.
  • Collaborates with Chief Operating Officer or designee to identify the need for and conduct in services for clinical staff.
  • Promotes and models collaborative interdisciplinary practice processes among care team. Ensures coordinated continuity of care and achievement of optimal outcomes; is a team and consensus builder.

Requirements


Responsible for the overall direction, coordination, and evaluation of Intake and Admissions teams. Directly supervises Intake and Admissions Teams. Carries out supervisory responsibilities in accordance with the organizations policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; and addressing complaints and resolving problems.
