Job Description

Position Purpose:

The Support Coordinator- Community Living has the primary responsibility of supporting the Assistant Director-Community Living to ensure quality services are provided and the level of care and dignity individuals in VersAbility’s homes and residential programs is maintained at optimal levels. This directly affects the image and reputation of VersAbility in the community and by extension, on the communities’ perception of people with disabilities.

Primary Duties:
  • Coordinates and facilitates residential admissions and discharge committee meetings. Maintains candidate pool list and referral list
  • Coordinates with contracted and other providers to ensure individual needs are being met
  • Provides accurate and timely documentation as required for Medicaid Waiver, Medicare, Social Security, ICF and licensing requirements
  • Administers admissions and discharges of individuals
including social work histories, plans, quarterlies, and yearly reports
  • Serves as a liaison with families, friends, and other service organizations
  • Read, interpret, implement, and understand policies, licensures, regulations, program requirements and state of the art methodologies of the
treatment of persons with developmental and other disabilities
  • Inspect residential homes for cleanliness, safety, homelike environment, and proper documentation
  • Ensure quality leisure activities and experiences for residents
  • Plans, coordinates, and conducts monthly QIDP meetings and ensures each is documented properly
  • Assists with staff training when requested
  • Review policies and procedures for compliance with regulations
  • Inspects proper procedures for medication administration in all homes
Assists in the implementation of new programs and services
  • Provide 24-hour on call status for staff to address issues related to support plans and/or behavioral issues
  • Interacts with families and friends of service recipients
  • Assists with reviews of documentation of all programs
  • Prepare reports as required
  • Assists Assistant Director to ensure home is continually in compliance with the standards and regulations of the Department of Licensure,
Medicaid Waiver, ICFMR. Department of Health, CARF, Department of Social Services and any other applicable bodies
  • Other related duties as assigned
  • Provide support for community integration efforts and HCBS Rights Compliance

Qualifications/Skills Knowledge & Abilities:

COVID-19 Considerations:

Proof of COVID-19 vaccinations is required for employment. Masks are required.

Education: Bachelors degree in human services, psychology or a related field required. Must be able to maintain CPR, CPI, first aid and medication administration certifications on an ongoing basis. Must have a safe driving record and a valid state drivers license

Experience: Two years experience working with adults with developmental and other disabilities, - or experience interpreting and implementing regulations, or designing and implementing programs, - or other equivalent experience that would prepare for this position.
