Overview Clinical Pharmacist Medicare D is a specialized pharmacist who works with Medicare Part D prescription drug plans to ensure that patients receive the best possible care. They are responsible for providing clinical services to patients, including medication reviews, drug utilization reviews, and patient education. They also work with physicians and other healthcare providers to ensure that the medications prescribed are appropriate and cost-effective. Detailed Job Description Clinical Pharmacist Medicare D is responsible for providing clinical services to patients, including medication reviews, drug utilization reviews, and patient education. They must be knowledgeable about the Medicare Part D prescription drug plans and be able to explain them to patients. They must also be able to identify potential drug interactions and provide appropriate recommendations. They must be able to work with physicians and other healthcare providers to ensure that the medications prescribed are appropriate and cost-effective. Job Skills Required• Knowledge of Medicare Part D prescription drug plans
• Ability to explain Medicare Part D plans to patients
• Ability to identify potential drug interactions

Job Qualifications
• Bachelor’s degree in pharmacy or related field
• Current state pharmacy license
• At least two years of experience in a clinical pharmacy setting
• Knowledge of Medicare Part D prescription drug plans
• Ability to explain Medicare Part D plans to patients
• Ability to identify potential drug interactions
• Ability to provide appropriate recommendations
• Ability to work with physicians and other healthcare providers

• Knowledge of Medicare Part D prescription drug plans
• Knowledge of drug interactions
• Knowledge of pharmacology
• Knowledge of medical terminology
• Knowledge of pharmacy laws and regulations
Job Experience
• At least two years of experience in a clinical pharmacy setting

• Experience providing patient education
Job Responsibilities
• Provide clinical services to patients, including medication reviews, drug utilization reviews, and patient education
• Explain Medicare Part D prescription drug plans to patients
• Identify potential drug interactions and provide appropriate recommendations

• Maintain patient records and document all services provided
