Client Care Navigator Jobs in Arizona

Provide patients with educational materials and execute correspondence to primary care physicians and specialists for new patient enrollments/appointments

Manage patient appointment cancelation and rescheduling as needed

Collaborate with Finance colleagues on patient billing issues

Manage the incoming queue of patient referrals

Strong organizational skills and ability to juggle multiple high-priority tasks and maintain a personal schedule

Flexibility to change course and take on additional responsibilities as the business requires

Provide personal care training for CAREGivers as part of continuous education.

Share on-call responsibilities outside of normal business hours to include support for office staff, clients, CAREGivers, and community partners.

Maintain regular attendance to execute job responsibilities

Conduct Family Education sessions as needed throughout year

Must demonstrate excellent oral and written communication skills and the ability to listen effectively

Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills

4 years education/experience; Masters Degree preferred

Provide education and support in treatment and recovery efforts.

Two years in Social Services field experienced with the target population

Should have excellent language and people skills and be well organized in work habits

Explain services to consumers, identify risk factors, and encourage consumers to participate.

Develop cooperative working relationships with community care providers.
