Description
Position Summary:
Conducts initial contact and coordinates services for HRSA RCORP Implementation grant
Essential Job Functions
- Maintain updated progress notes of services, referrals, and accomplishments in consumer files in a timely manner as outlined by HRSA RCORP grant Standards and Guidelines
- Explain services to consumers, identify risk factors, and encourage consumers to participate.
- Act as a liaison/advocate to community services and primary health care providers.
- Ability to maintain a caseload of approximately 50 consumers per month.
- Develop cooperative working relationships with community care providers.
- Provide education and support in treatment and recovery efforts.
- Attend staff meetings, training and in-service as assigned by the Program Director or required by agency, or funding source
- Participate in Performance Improvement activities in an ongoing basis.
- Other duties as assigned
- Provide care coordination and referrals.
- Responds to referrals of assigned program participants in a timely manner to complete initial assessment, treatment plan and follow up as needed.
Essential Qualifications:
Certifications/Licenses
- As mandated by Presidential Order all employees of WestCare Foundation, Inc. and all affiliates of WestCare Foundation, Inc. will be required to be fully vaccinated against COVID-19 effective 11/1/2021. Employees that have requested a Medical or Religious Exemption, which has been approved by Human Resources will be required to test for the virus weekly
- Able to pass Arizona Central Registry background check
- Possess or able to obtain Arizona Level 1 Fingerprint clearance card
- During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you
- 4 years education/experience; Masters Degree preferred
- Comfortable working with mental health and drug using population
- Ability to read, analyze, and interpret the most complex documents
- Two years in Social Services field experienced with the target population
- Bi-lingual is a plus
- Should have excellent language and people skills and be well organized in work habits
- Ability to respond effectively to the most sensitive inquiries or complaints
- Normal work hours are from a.m. p.m. Regular attendance is required. Working Overtime is required and essential to perform job duties at times.
